LVEF 45% Classification and Management
An LVEF of 45% classifies as heart failure with mildly reduced ejection fraction (HFmrEF), and you should initiate guideline-directed medical therapy (GDMT) similar to HFrEF, including SGLT2 inhibitors, ACE inhibitors/ARNi, beta-blockers, and mineralocorticoid receptor antagonists. 1, 2
Classification
LVEF of 45% falls into the HFmrEF category (LVEF 41-49%) according to the 2022 AHA/ACC/HFSA guidelines 1, 2. This is a critical distinction because:
- HFrEF is defined as LVEF ≤40%
- HFmrEF is defined as LVEF 41-49%
- HFpEF is defined as LVEF ≥50%
Clinical Significance
Prognostic Implications
LVEF of 45% carries important prognostic information. Below 45%, each 10% reduction in LVEF increases all-cause mortality by 39% 3. However, once LVEF exceeds 45%, further increases do not substantially modify cardiovascular risk 3. This makes your patient's LVEF of 45% a critical threshold value.
Dynamic Nature - Critical Consideration
HFmrEF is not a static diagnosis. Patients with LVEF 45% exist on a dynamic trajectory 2, 4:
- 43% improve to HFpEF (LVEF ≥50%) within one year
- 17% deteriorate to HFrEF (LVEF <40%) within one year 4
Patients who worsen to HFrEF have a 7-fold increased risk of all-cause mortality compared to those who improve 4. Therefore, serial LVEF measurements are essential - a single measurement at one time point is inadequate 2.
Management Algorithm
Step 1: Confirm Diagnosis of HFmrEF
Beyond LVEF 41-49%, you must document:
- Elevated natriuretic peptides (BNP or NT-proBNP)
- Evidence of increased LV filling pressures:
- Echocardiographic E/e' ≥15, OR
- Invasive hemodynamic measurements, OR
- Structural heart disease (increased left atrial volume index or LV mass index) 2
Step 2: Initiate Quadruple Therapy
Start all four foundational drug classes rapidly 5:
- SGLT2 inhibitors (dapagliflozin or empagliflozin) - initiation does NOT require waiting for LVEF confirmation 6
- ACE inhibitor/ARB or ARNi (angiotensin receptor-neprilysin inhibitor)
- Beta-blocker (carvedilol, metoprolol succinate, or bisoprolol)
- Mineralocorticoid receptor antagonist (spironolactone or eplerenone) - initiation does NOT require waiting for LVEF confirmation 6
Plus diuretics for volume overload symptoms 5
Step 3: Rapid Uptitration
Do not delay. Contemporary management emphasizes early initiation and rapid uptitration to target doses 5. The evidence for benefit in HFrEF (LVEF ≤40%) extends to the HFmrEF range, and these patients show treatment responses closer to HFrEF than HFpEF 7.
Step 4: Serial LVEF Monitoring
Reassess LVEF at 3-6 months and then annually 2. The trajectory determines ongoing management:
- If LVEF improves to >40%: Classify as HFimpEF (heart failure with improved EF) and continue all HFrEF therapies - do NOT discontinue medications as EF can decrease after withdrawal 2
- If LVEF worsens to <40%: Reclassify as HFrEF and consider ICD evaluation if LVEF remains ≤35% after 3 months of optimal medical therapy
- If LVEF remains 41-49%: Continue current management and monitor
Critical Pitfalls to Avoid
Do NOT wait for "optimal" LVEF measurement before starting therapy. SGLT2 inhibitors and MRAs can be initiated based on elevated natriuretic peptides alone in suspected de novo heart failure 6
Do NOT discontinue medications if LVEF improves. Patients with HFimpEF who stop therapy frequently experience EF decline 2
Do NOT rely on a single LVEF measurement. The trajectory over time is more important than the absolute value 2, 6
Do NOT assume preserved systolic function means low risk. LVEF measurement reproducibility is poor, and its prognostic value lessens above 45% 6
Do NOT undertreated based on "borderline" EF. Only 24% of HFrEF patients receive guideline-recommended triple therapy in real-world practice - this represents a major treatment gap 8
Predictors of Worsening
Lower baseline LVEF (each 1% decrease) and reduced tricuspid annular plane systolic excursion independently predict deterioration to HFrEF 4. Monitor these parameters closely.
