SGLT2 Inhibitors: Clinical Overview
SGLT2 inhibitors (canagliflozin, dapagliflozin, empagliflozin, ertugliflozin) are now foundational therapies that reduce cardiovascular death, heart failure hospitalization, and kidney disease progression—benefits that extend well beyond glucose control and apply even to patients without diabetes.
Indications
Type 2 Diabetes
All four agents are approved to improve glycemic control as adjunct to diet and exercise 1.
Cardiovascular Disease
- Empagliflozin: Reduces CV death in adults with T2D and established CVD 1
- Canagliflozin: Reduces risk of MI, stroke, or CV death in adults with T2D and CVD 1
- Dapagliflozin: Reduces hospitalization for HF in adults with T2D and established CVD or multiple CV risk factors 1
Heart Failure with Reduced Ejection Fraction (HFrEF)
Both dapagliflozin and empagliflozin are strongly recommended for all patients with HFrEF, regardless of diabetes status 2. Dapagliflozin demonstrated a 26% reduction in the composite endpoint of worsening HF or CV death (HR 0.74; 95% CI 0.65-0.85) 1. This benefit was consistent in patients without diabetes (HR 0.73) and with diabetes (HR 0.75) 3.
Chronic Kidney Disease
- Canagliflozin: Reduces risk of end-stage kidney disease, doubling of serum creatinine, CV death, and HF hospitalization in patients with T2D and diabetic nephropathy with albuminuria 1
- Dapagliflozin: Approved for CKD with eGFR 25-75 mL/min/1.73 m² and proteinuria, reducing sustained eGFR decline ≥50%, end-stage kidney disease, or death from renal/CV causes by 44% (HR 0.56; 95% CI 0.45-0.68) 2
Dosing Regimens
Standard Dosing
- Canagliflozin: 100 mg PO daily 1, 4
- Dapagliflozin: 10 mg PO daily (5 mg starting dose for glycemic control only) 1, 5
- Empagliflozin: 10 mg PO daily 1, 4
- Ertugliflozin: 5 mg PO daily 6
Renal Dose Adjustments
For Glycemic Control:
- Canagliflozin:
- Dapagliflozin:
- Empagliflozin:
Critical Clinical Point: The glucose-lowering effect diminishes below eGFR 45 mL/min/1.73 m², but cardiovascular and renal benefits persist down to eGFR 30 mL/min/1.73 m² 4. For HF and CKD indications, continue therapy until dialysis or transplantation 7.
For Heart Failure/CKD Indications:
- Dapagliflozin: Can initiate down to eGFR 25 mL/min/1.73 m² for HF or CKD indications; if eGFR <25, do not initiate but may continue 10 mg in patients already on therapy 6
- Empagliflozin/Canagliflozin: EMPEROR-Reduced excluded eGFR <20; DAPA-HF excluded eGFR <30 2
Contraindications
Absolute Contraindications
- History of serious hypersensitivity reaction to the drug 1
- Type 1 diabetes 2
- Dialysis (all agents) 1
- Pregnancy or breastfeeding (dapagliflozin specifically noted) 1
Relative Contraindications Based on Renal Function
See dosing section above for specific eGFR cutoffs by indication 1, 6.
Cautions and Special Populations
Surgical Considerations
Discontinue at least 3 days before planned surgery to prevent postoperative diabetic ketoacidosis 1. Also hold during critical illness or prolonged fasting 7.
Volume Depletion
May contribute to intravascular volume contraction. Assess volume status before initiation and consider stopping or reducing diuretic dose if applicable 1. Monitor blood pressure, particularly when ill or fasting 7.
Hypoglycemia Risk
If HbA1c is well-controlled at baseline or patient has history of frequent hypoglycemic events, wean or stop sulfonylurea/glinide and consider reducing total daily insulin dose by approximately 20% when starting SGLT2 inhibitor 1.
Amputation Risk (Canagliflozin)
Use with caution in patients with prior amputation, severe peripheral neuropathy, severe peripheral vascular disease, or active diabetic foot ulcers/soft tissue infections 1. Possible increased risk of bone fractures with canagliflozin 1.
Monitoring Recommendations
Renal Function
- Assess eGFR prior to initiation and then as clinically indicated 5
- Expect temporary reduction in eGFR up to 15%, which generally resolves within 1-3 months 2
- Increased monitoring warranted in those at risk for acute kidney injury 2
- Continue monitoring kidney function upon initiation, particularly in patients on loop diuretics 8
Common Adverse Effects to Monitor
Genital Mycotic Infections (Most Common):
- Women: 10-15% risk 2
- Highest risk: women, those with previous GMIs, uncircumcised men 2
- Typically managed with antifungal drugs without discontinuation 2
- Mitigate risk with genital hygiene; avoid use in high-risk individuals 7
Urinary Tract Infections:
- Monitor for symptoms 1
Diabetic Ketoacidosis (Rare but Serious):
- Risk increased in insulin-deficient states (rare in T2D) 7
- Discontinue, evaluate, and treat promptly if suspected 7
- Be aware of predisposing risk factors and clinical presentations, including euglycemic DKA 7
- Implement sick-day planning 2
Necrotizing Fasciitis of Perineum (Fournier Gangrene):
- Rare but requires prompt treatment if suspected 7
Lower Limb Complications:
- Monitor for ulcerations, soft tissue infections, and amputations (particularly with canagliflozin) 1
Clinical Implementation Algorithm
Step 1: Identify Indication Priority
- HFrEF (with or without diabetes) → Dapagliflozin or empagliflozin 2
- T2D + established ASCVD → Any SGLT2i with CV outcome benefit 2, 9
- T2D + albuminuric CKD → Canagliflozin or dapagliflozin 2
- T2D + age >50 with CV risk factors → Dapagliflozin 2
Step 2: Check Renal Function
- eGFR ≥45: All agents appropriate for all indications
- eGFR 30-44: Continue for CV/renal benefit; limited glucose-lowering effect 7
- eGFR 25-29: Dapagliflozin for HF/CKD indications only 6
- eGFR <25: Do not initiate; may continue dapagliflozin in established HF/CKD 6
Step 3: Assess Contraindications
- Type 1 diabetes, dialysis, pregnancy → Do not use
- Prior amputation/severe PVD (for canagliflozin) → Consider alternative agent
Step 4: Optimize Concomitant Therapy
- Reduce insulin by ~20% if applicable 1
- Wean sulfonylurea/glinide if HbA1c well-controlled 1
- Assess volume status; adjust diuretics if needed 1
Step 5: Initiate and Monitor
- Start standard dose (see dosing section)
- Counsel on genital hygiene and sick-day management 2
- Monitor eGFR at initiation and as clinically indicated 5
- Expect modest eGFR decline initially; do not discontinue unless persistent decline 2
Key Clinical Pearls
Start Early: In HFrEF trials, SGLT2 inhibitors were added to maximally tolerated guideline-directed medical therapy, but given the significant residual risk, it is reasonable to start early in the disease course 2.
Independent of Glycemic Control: Treatment decisions should be made independent of background therapy, current glycemic control levels, or individualized treatment goals when used for CV/renal indications 10, 9.
Class Effect with Nuances: While cardiovascular and renal benefits appear to be a class effect, specific agents have demonstrated benefits in specific populations based on trial data 1, 2.
Sick-Day Management: Hold SGLT2 inhibitors during acute illness, prolonged fasting, or before surgery to minimize DKA risk 2, 7.
