What is the Metavir Staging System?
The METAVIR scoring system is a standardized histological classification that grades liver fibrosis on a 5-point scale (F0-F4) and necro-inflammatory activity on a 4-point scale (A0-A3), primarily used to assess disease severity in chronic hepatitis C patients. 1, 2
Fibrosis Staging (F0-F4)
The METAVIR fibrosis scale categorizes liver scarring as follows:
- F0: No fibrosis
- F1: Portal fibrosis without septa
- F2: Portal fibrosis with few septa (significant fibrosis)
- F3: Numerous septa without cirrhosis (advanced fibrosis/bridging fibrosis)
- F4: Cirrhosis
Necro-inflammatory Activity Grading (A0-A3)
The activity component assesses hepatic inflammation:
- A0: No activity
- A1: Mild activity
- A2: Moderate activity
- A3: Severe activity
Clinical Significance and Decision Points
The distinction between F0-F2 versus F3-F4 is clinically critical, as it fundamentally changes management:
F0-F2 (No to significant fibrosis): After achieving sustained virological response (SVR), no additional follow-up is required 2
F3 (Advanced fibrosis): Requires post-treatment surveillance for hepatocellular carcinoma (HCC) with twice-yearly abdominal imaging, even after SVR 1, 2
F4 (Cirrhosis): Mandates HCC surveillance with twice-yearly imaging, continued endoscopic screening for varices, and potential adjustment of treatment regimens 1, 2
Important Caveats
Necroinflammatory activity and fibrosis do not progress in parallel. Studies demonstrate that fibrosis can advance even with mild inflammatory activity, particularly in hepatitis C where the disease shows greater fibrogenic potential compared to hepatitis B 4. The median time to cirrhosis progression is approximately 30 years but varies dramatically based on host factors—ranging from 13 years in men infected after age 40 to 42 years in women without alcohol consumption infected before age 40 3.
METAVIR was designed for chronic hepatitis assessment, not congestive hepatopathy. While it shows perfect agreement with specialized scores like the Congestive Hepatic Fibrosis Score (CHFS) in high-grade fibrosis (F3-F4), agreement is only substantial in early-stage disease due to different architectural emphases (portal-based versus centrilobular) 5.
Non-invasive Assessment Context
While liver biopsy using METAVIR remains the reference standard, non-invasive methods (FibroScan, APRI, FIB-4) should be used for initial fibrosis staging 1. However, these methods are unreliable for assessing fibrosis after therapy and should not be used in that setting 1.
