Tamiflu (Oseltamivir) Dosing and Administration
For adults and adolescents 13 years and older, take Tamiflu 75 mg twice daily for 5 days for treatment, starting within 2 days of symptom onset; for prevention, take 75 mg once daily for at least 10 days after exposure. 1
How to Take Tamiflu
Tamiflu can be taken with or without food, but taking it with food may reduce nausea and vomiting, which are the most common side effects 2, 1. The medication is administered orally and does not require any special timing relative to meals, though tolerability is enhanced when taken with food 2.
Treatment Dosing (5 Days)
Adults and Adolescents (≥13 years)
Children (≥1 year) - Weight-Based Dosing
The dosing is based on body weight 2, 1:
- ≤15 kg (≤33 lbs): 30 mg twice daily
- 16-23 kg (34-51 lbs): 45 mg twice daily
- 24-40 kg (52-88 lbs): 60 mg twice daily
- ≥41 kg (≥89 lbs): 75 mg twice daily
Infants (3-12 months)
Infants (<3 months)
Prevention (Prophylaxis) Dosing
After Exposure to Infected Person
- Adults/Adolescents: 75 mg once daily for 10 days, starting within 2 days of exposure 1
- Children: Same weight-based doses as treatment, but once daily instead of twice daily 2, 1
During Community Outbreak
- Adults: 75 mg once daily for up to 6 weeks (up to 12 weeks in immunocompromised patients) 1
- Children: Same weight-based doses once daily for up to 6 weeks 1
Formulations and Administration Tips
Tamiflu is available as:
For patients who cannot swallow capsules, the oral suspension is preferred 1. If suspension is unavailable, capsules can be opened and mixed with sweetened liquids such as chocolate syrup, corn syrup, caramel topping, or light brown sugar dissolved in water to mask the bitter taste 2, 1.
Special Populations
Renal Impairment
For creatinine clearance 10-30 mL/min 2, 1:
- Treatment: 75 mg once daily for 5 days (instead of twice daily)
- Prevention: 30 mg once daily OR 75 mg every other day for 10 days
Premature Infants
Current dosing recommendations are NOT appropriate for premature infants due to immature renal function that may lead to dangerously high drug concentrations 2, 4. Very limited data suggest approximately 1.7 mg/kg twice daily, but this is insufficient for firm recommendations 4.
Critical Timing Considerations
The earlier Tamiflu is started, the better the outcome. Starting within 12 hours of fever onset reduces illness duration by 3.1 days (41%) more than starting at 48 hours 3. Benefits are progressive at every earlier time point of intervention 3. While the FDA label specifies starting within 2 days of symptoms 1, evidence suggests treatment initiated even within 5 days may benefit critically ill patients with H1N1 6.
Common Pitfalls to Avoid
- Don't skip doses or stop early: Complete the full 5-day treatment course even if feeling better
- Don't delay treatment: Efficacy decreases significantly after 48 hours from symptom onset 1, 3
- Don't use in children <1 year without careful consideration: Not FDA-approved for this age group, though emergency use data exists 4, 1
- Don't forget renal dose adjustments: Failure to reduce dose in renal impairment can lead to toxicity 2, 1
- Don't confuse treatment and prevention dosing: Prevention uses half the daily dose (once daily vs twice daily) 1
Important Limitations
Tamiflu is not a substitute for annual influenza vaccination 1. It only works against influenza viruses types A and B, not other respiratory infections 1. Resistance can emerge, particularly in critically ill patients 7, so prescribers should consider local resistance patterns when deciding to use Tamiflu 1.
