Initiating Dose of Eliquis for Atrial Fibrillation
For most patients with atrial fibrillation, start Eliquis at 5 mg orally twice daily. 1
Standard Dosing
The default initiating dose is 5 mg twice daily for stroke prevention in nonvalvular atrial fibrillation. This is the dose used in the pivotal ARISTOTLE trial that demonstrated superiority over warfarin for reducing stroke and systemic embolism (hazard ratio 0.79, p=0.01) with significantly lower rates of major bleeding. 1
Dose Reduction Criteria
Reduce to 2.5 mg twice daily ONLY if the patient meets at least TWO of the following three criteria: 1
- Age ≥80 years
- Body weight ≤60 kg
- Serum creatinine ≥1.5 mg/dL
This is critical: patients with only ONE of these characteristics should receive the standard 5 mg twice daily dose. The ARISTOTLE trial specifically demonstrated that patients with a single dose-reduction criterion maintained consistent efficacy and safety with 5 mg twice daily compared to warfarin, with no increased bleeding risk. 2
Common Prescribing Errors to Avoid
Do not inappropriately dose-reduce. Real-world data consistently shows that 40-60% of patients receiving 2.5 mg twice daily do not meet the dose-reduction criteria. 3, 4 This inappropriate underdosing exposes patients to increased thromboembolic risk without additional safety benefit. In one study, 41% of patients newly initiated on 2.5 mg for atrial fibrillation were receiving a potentially inappropriate dose. 4
Patients with isolated advanced age (but <80 years), isolated low-normal weight (but >60 kg), or isolated mild renal dysfunction (creatinine <1.5 mg/dL) should receive 5 mg twice daily. 2
Timing of First Dose
Initiate the first dose at any time once the decision to anticoagulate is made—there is no specific loading dose or timing requirement for atrial fibrillation. 1
Renal Function Considerations
For patients with CrCl 15-50 mL/min who also meet the two-criterion rule above, use 2.5 mg twice daily. However, if they have CrCl 15-50 mL/min but do NOT meet two of the three dose-reduction criteria, the standard 5 mg twice daily dose remains appropriate per FDA labeling. 5, 1
Avoid Eliquis entirely if CrCl <15 mL/min or if the patient is on dialysis—this was an exclusion criterion in ARISTOTLE and lacks safety data. 5
