Additional Glucose-Lowering Options at eGFR 40
At eGFR 40 mL/min/1.73 m², you have several excellent medication options beyond insulin—your patient is already on optimal therapy with tirzepatide (Mounjaro) and dapagliflozin (Farxiga), and you can add metformin, DPP-4 inhibitors, or consider insulin if needed for glycemic control.
Current Regimen Assessment
Your patient's current medications are well-chosen for CKD:
Dapagliflozin (Farxiga): At eGFR 40, this is appropriate and should be continued at 10 mg daily 1. While glucose-lowering efficacy diminishes below eGFR 45, the cardiovascular and kidney protective benefits persist, making continuation essential until dialysis 2, 3.
Tirzepatide (Mounjaro): Requires no dose adjustment at eGFR 40 1, 4. Recent evidence shows tirzepatide reduces albuminuria and slows eGFR decline, with particular benefit in CKD populations 5, 6. The SURPASS-CVOT trial demonstrated a 23% reduction in major kidney events compared to dulaglutide 6.
Additional Medication Options
First-Line Addition: Metformin
Add metformin at reduced dose (maximum 1000 mg/day) if not already prescribed 1, 7.
- At eGFR 30-44 (Stage 3b CKD), metformin dose should be halved to maximum 1000 mg daily 1
- Monitor eGFR every 3-6 months 8, 7
- Discontinue if eGFR drops below 30 3, 8
- Check vitamin B12 levels with long-term use 8
Second-Line Addition: DPP-4 Inhibitors
If additional glucose lowering is needed beyond metformin:
Linagliptin is preferred—no dose adjustment required at any eGFR level 1
Alternative DPP-4 inhibitors with dose adjustments at eGFR 40:
- Sitagliptin: Maximum 50 mg daily 1
- Alogliptin: Maximum 12.5 mg daily 1
- Saxagliptin: Maximum 2.5 mg daily 1
When to Consider Insulin
Insulin becomes necessary when 3:
- Target HbA1c not achieved with oral/injectable agents
- Patient has significant insulin deficiency
- eGFR drops below 30 (when metformin must be stopped)
Critical caveat: Initiate and titrate insulin conservatively at eGFR 40 to avoid hypoglycemia, as CKD increases hypoglycemia risk 1. Patients with CKD have reduced insulin clearance and are at higher risk for severe hypoglycemia 3.
Medications to AVOID at eGFR 40
- Glyburide: Not recommended at any CKD stage 1
- Exenatide (once-weekly): Avoid at eGFR <45 1
- Empagliflozin/Ertugliflozin: Not recommended for initiation at eGFR <45 1
Important Safety Considerations
Combination Therapy Risks
If combining tirzepatide with insulin or sulfonylureas, reduce doses of these agents to prevent hypoglycemia 9, 10. The most common hypoglycemia in tirzepatide studies occurred in insulin users 11.
SGLT2 Inhibitor + GLP-1 RA Combination
Monitor for euglycemic diabetic ketoacidosis (EDKA) when using dapagliflozin with tirzepatide 12. Risk factors include:
- Acute illness or infection
- Reduced food intake (tirzepatide causes appetite suppression in 14% of patients 12)
- Surgical procedures
Sick-day management: Temporarily discontinue both dapagliflozin and tirzepatide during acute illness, fasting, or surgery 10, 12.
Volume Status Monitoring
With dapagliflozin at eGFR 40:
- Expect modest initial eGFR decline (hemodynamic, reversible) 13
- Monitor volume status—adjust diuretics if needed 13, 10
- Continue dapagliflozin even if eGFR drops below 30, as long as tolerated and dialysis not imminent 13, 1
Practical Algorithm
- If HbA1c not at goal: Add metformin 500 mg daily, titrate to maximum 1000 mg/day
- If still not at goal with metformin: Add linagliptin 5 mg daily (no dose adjustment needed)
- If still not at goal: Consider basal insulin with conservative titration
- Monitor: eGFR every 3-6 months, adjust medications as kidney function changes 8, 7
You are NOT limited to insulin at eGFR 40—this patient has multiple safe and effective options that provide both glycemic control and kidney protection 3, 7.
