Contraindications for Midodrine
Midodrine is absolutely contraindicated in patients with severe organic heart disease, acute renal disease, urinary retention, pheochromocytoma, thyrotoxicosis, and persistent/excessive supine hypertension 1.
Absolute Contraindications (FDA Drug Label)
The FDA drug label explicitly lists the following as contraindications 1:
- Severe organic heart disease
- Acute renal disease
- Urinary retention
- Pheochromocytoma
- Thyrotoxicosis
- Persistent and excessive supine hypertension
Critical Clinical Context
Supine Hypertension - The Most Important Limiting Factor
Supine hypertension is both a contraindication and the most common serious adverse effect of midodrine. The drug can cause marked elevation of supine blood pressure exceeding 200 mmHg systolic 1. This is particularly problematic because:
- It occurs in approximately 7.3% of patients 1
- Guidelines specifically note that when supine hypertension is present, other medications should be used before fludrocortisone (and by extension, midodrine) 2
- Patients must be counseled to avoid lying flat for 4 hours after dosing
Urinary System Contraindications
Midodrine acts on alpha-adrenergic receptors of the bladder neck, causing:
- Urinary urgency (13.4% of patients)
- Urinary retention (13.4% of patients)
- Increased urinary frequency 1
Therefore, any patient with pre-existing urinary retention or significant prostatic hypertrophy should not receive midodrine.
Cardiovascular Contraindications
The "severe organic heart disease" contraindication requires clinical judgment but should include:
- Severe coronary artery disease
- Uncontrolled heart failure
- Severe valvular disease
- Recent myocardial infarction
Endocrine Contraindications
Both pheochromocytoma and thyrotoxicosis are absolute contraindications because midodrine's alpha-agonist effects would dangerously exacerbate catecholamine excess states 1.
Common Pitfalls in Practice
Real-world data shows that 55.7% of patients prescribed midodrine would have been excluded if physicians strictly followed the contraindications listed in the drug label 3. This inappropriate prescribing was associated with a 34% adverse event rate and required drug discontinuation in 15.5% of cases 3.
Key clinical pearl: Always check supine blood pressure before prescribing and educate patients to sleep with the head of the bed elevated to mitigate supine hypertension risk.
