Testosterone Does Not Cause Rhinorrhea
Testosterone replacement therapy (TRT) is not associated with rhinorrhea as a recognized adverse effect. Based on comprehensive guideline evidence from the New England Journal of Medicine and the Endocrine Society, rhinorrhea is not listed among the documented side effects of testosterone therapy 1, 2.
Documented Side Effects of TRT
The well-established adverse effects of testosterone therapy include 1:
Common Dermatologic Effects
- Skin reactions: Erythema and pruritus occur in up to 66% with patches, but only 5% with gel preparations
- Acne and oily skin: Minor but recognized effects
- Local injection site reactions: Pain, soreness, bruising, erythema, swelling, nodules, or furuncles with intramuscular formulations
Respiratory Effects
The only respiratory complication documented with TRT is sleep apnea exacerbation 1, 3. This occurs through central mechanisms rather than anatomical airway changes, typically in men with pre-existing risk factors receiving higher parenteral doses. Importantly, this manifests as disordered breathing during sleep—not nasal symptoms like rhinorrhea.
Other Established Side Effects
- Erythrocytosis: 3-18% with transdermal, up to 44% with injections
- Breast tenderness and gynecomastia: Rare
- Testicular atrophy: Common and expected
- Fluid retention: Uncommon and mild
- Prostate volume increase: Without symptomatic worsening
Why Rhinorrhea Is Not Associated with TRT
The evidence is clear that rhinorrhea is specifically associated with estrogen-based hormone therapy, not testosterone 4, 5. Studies demonstrate that:
- Menopausal hormone therapy (estrogen-based) increases non-allergic rhinitis and rhinorrhea in postmenopausal women 4
- Combined oral contraceptives cause nasal mucosal changes through estrogen receptors 5
- Testosterone acts through entirely different receptor mechanisms and does not produce these nasal effects
Clinical Pitfall to Avoid
If a patient on TRT reports rhinorrhea, do not attribute it to the testosterone. Instead, investigate alternative causes:
- Allergic rhinitis
- Viral upper respiratory infection
- Vasomotor rhinitis
- Medication side effects from other drugs
- Environmental irritants
The comprehensive FDA labeling for testosterone products does not list rhinorrhea as an adverse effect 6, and the most recent systematic reviews of TRT adverse effects make no mention of nasal symptoms 7, 8, 9.
