Tofacitinib Contraindications
Tofacitinib has no absolute contraindications listed in the FDA label, but there are critical warnings and situations where it should not be used or used with extreme caution. 1
Key Warnings That Function as Practical Contraindications
1. Severe Active or Recurrent Infections
- Do not initiate tofacitinib during active serious infections, including localized infections 2, 1
- This includes:
- Active tuberculosis or untreated latent TB
- Opportunistic infections (cryptococcosis, histoplasmosis, pneumocystosis, disseminated herpes zoster)
- Chronic/recurrent bacterial infections
- Sepsis
Critical pitfall: The 2025 expert consensus changed "chronic" to "recurrent" infection because active infections may recur without being chronic—both scenarios warrant extreme caution 2. Screen for latent TB before initiating therapy and monitor closely during treatment 1.
2. Severe Organ Dysfunction
- Severe hepatic impairment (Child-Pugh C): Tofacitinib is not recommended in any patient population 3, 1
- Severe renal impairment (CrCl <30 mL/min): Reduce dose to 5 mg once daily; for patients on hemodialysis, administer after dialysis 3
3. Hematologic Thresholds - Do Not Initiate If:
- Absolute lymphocyte count <500 cells/mm³
- Absolute neutrophil count <1,000 cells/mm³
- Hemoglobin <9 g/dL 1
Once initiated, discontinue if: Lymphocyte count drops below 500 cells/mm³ or ANC falls below 500 cells/mm³ 1
4. Pregnancy and Lactation
- Avoid tofacitinib during preconception, pregnancy, and breastfeeding 4
- Tofacitinib is teratogenic in animal studies (rats and rabbits) and affects parturition 3
- Contraception is advised for both female and male patients 3
- Advise patients not to breastfeed while on tofacitinib 1
5. High-Risk Cardiovascular and Thrombotic Situations
While not absolute contraindications, use extreme caution or avoid in:
- Patients ≥65 years with cardiovascular risk factors (per EMA, use only if no alternative) 3
- History of thrombotic episodes or high risk for venous thromboembolism 4
- Uncontrolled cardiac risk factors 4
Evidence note: The ORAL Surveillance trial showed increased serious infections and cardiovascular events in elderly RA patients with CV risk factors on tofacitinib 10 mg twice daily compared to TNF inhibitors 3.
6. Active or Recent Malignancy
- Shared decision-making required considering timing of past malignancy, uncontrolled malignancy, or ongoing chemotherapy including checkpoint inhibitors 3
- Most clinical trials excluded patients with malignancy within 5 years prior to enrollment 3
- While registries show no clear malignancy signal, the 39-week monkey study showed lymphomas at high exposures 1
Dosage Adjustments Required (Not Contraindications, But Critical)
When Using Strong CYP Inhibitors:
- Strong CYP3A4 inhibitors (ketoconazole): Reduce dose
- Moderate CYP3A4 + strong CYP2C19 inhibitors (fluconazole): Reduce dose 3, 1
When Using Rifampicin:
Special Population Considerations
Elderly patients (≥60 years): Higher risk of serious infections and SAEs; older age is an independent risk factor 5. Use only if no alternative available per EMA guidance 3.
Patients with prior endemic exposure: Exercise caution in those who have resided or traveled in areas endemic for TB or mycoses 1.
Ulcerative colitis patients: The 10 mg twice daily dose carries greater infection risk than 5 mg twice daily, including opportunistic herpes zoster infections (meningoencephalitis, disseminated cutaneous) 1.
Monitoring Before Initiation
- Screen for latent and active TB
- Test for hepatitis B
- Complete baseline labs: CBC with differential, liver enzymes, lipid panel
- Assess cardiovascular risk factors
- Ideally complete adult vaccinations including herpes zoster before starting (live vaccines contraindicated during therapy) 4, 1
Bottom line: While tofacitinib has no FDA-listed absolute contraindications, severe active infections, severe hepatic impairment (Child-Pugh C), critical cytopenias, and pregnancy represent situations where the drug should not be used. Age ≥65 with CV risk factors requires careful risk-benefit assessment with preference for alternatives when available.
