Ejaculation Would Help Remove Kidney Stones Past the UPJ
For stones in the distal ureter (past the UPJ), ejaculation through sexual intercourse or masturbation 3-4 times per week significantly increases stone passage rates and should be recommended as an adjunctive therapy alongside standard medical management.
Evidence-Based Recommendation
While current major guidelines 1, 2 do not specifically address sexual activity for stone passage, they establish that distal ureteral stones ≤10 mm have spontaneous passage rates of 68-83% and recommend medical expulsive therapy (MET) with alpha-blockers for approximately 30 days 2. Sexual stimulation can be viewed as an effective adjunct to this conservative approach.
Supporting Research Evidence
The most recent and comprehensive meta-analysis 3 from 2024, analyzing 535 patients across six randomized controlled trials, demonstrates:
- 2-week stone expulsion rate: 77% higher with sexual stimulation vs placebo (RR = 1.77)
- 4-week stone expulsion rate: 48% higher with sexual stimulation vs placebo (RR = 1.48)
- Faster expulsion time: 3.74 days shorter on average
- Reduced renal colic attacks: Significantly fewer pain episodes
Mechanism and Clinical Application
Sexual stimulation works through several mechanisms:
- Smooth muscle relaxation in the ureter
- Increased ureteral peristalsis
- Hormonal effects that may facilitate stone passage
Practical recommendation: Advise patients with distal ureteral stones (5-10 mm) to engage in sexual intercourse or masturbation 3-4 times weekly while on standard MET with alpha-blockers 4, 5.
Gender-Specific Evidence
- Men: Masturbation showed equivalent efficacy to tamsulosin (81.4% vs 80.5% passage rates) 4
- Women: Sexual intercourse increased 2-week passage rates from 51.4% to 80% 5
Important Caveats
This recommendation applies specifically to:
- Distal ureteral stones (stones that have already passed the UPJ and are in the lower ureter)
- Stones 5-10 mm in size
- Patients with tolerable pain who can safely engage in sexual activity
- Stones without complications (no infection, preserved renal function, manageable pain)
Do not recommend if:
- Active urinary tract infection present
- Refractory pain requiring urgent intervention
- Declining renal function
- Stone causing complete obstruction
Integration with Standard Care
Sexual stimulation should be offered as an adjunct to, not replacement for, standard medical therapy 2:
- Start alpha-blocker (tamsulosin 0.4 mg daily)
- Add sexual activity recommendation (3-4 times weekly)
- Continue for up to 30 days
- Monitor for stone passage with imaging
If no passage occurs after 30 days or complications develop, proceed to ureteroscopy as per standard algorithms 1, 2.
