Apixaban (Eliquis) Prophylactic Dosing
The prophylactic dose of apixaban is 2.5 mg orally twice daily, initiated 12-24 hours after surgery for VTE prevention following hip or knee replacement. 1
FDA-Approved Prophylactic Indications and Dosing
Post-Orthopedic Surgery VTE Prophylaxis
- Dose: 2.5 mg orally twice daily
- Timing: Start 12-24 hours after surgery
- Duration:
This regimen demonstrated superior efficacy compared to enoxaparin 40 mg once daily in the ADVANCE-2 and ADVANCE-3 trials, with lower bleeding rates 2.
Extended VTE Prevention (After Initial Treatment)
- Dose: 2.5 mg orally twice daily
- Indication: After completing at least 6 months of anticoagulation for DVT/PE 1
- Evidence: This reduced dose significantly reduces recurrent VTE without increasing major bleeding compared to placebo 2
Cancer-Associated VTE Prophylaxis
For ambulatory cancer patients at intermediate-to-high VTE risk (Khorana score ≥2), apixaban 2.5 mg twice daily is recommended as primary prophylaxis 3, 4.
- Specifically indicated for pancreatic cancer patients on systemic therapy 3
- Must have low bleeding risk 3
- Recent evidence (API-CAT trial, 2025) supports reduced-dose apixaban (2.5 mg twice daily) for extended secondary prevention in cancer-associated VTE, showing 25% reduction in clinically relevant bleeding versus full-dose 5, 6
Important Caveats
When Prophylactic Dosing is NOT Appropriate
Do not use 2.5 mg twice daily for acute VTE treatment - this requires therapeutic dosing (10 mg twice daily for 7 days, then 5 mg twice daily) 1.
Hospitalized Medical Patients
Apixaban is NOT recommended for routine thromboprophylaxis in hospitalized medical patients. The ADOPT trial showed that extended prophylaxis with apixaban 2.5 mg twice daily was not superior to shorter-course enoxaparin and resulted in significantly more major bleeding (0.47% vs 0.19%, RR 2.58) 2, 7. Use LMWH or fondaparinux instead 8, 3.
Dose Adjustments
No dose adjustment is needed for the 2.5 mg twice daily prophylactic regimen - this is already the reduced dose. However:
- Avoid in patients already on combined P-gp and strong CYP3A4 inhibitors (ketoconazole, ritonavir) 1
- Consider avoiding in severe renal impairment (CrCl <15 mL/min) 9
Perioperative Management
When patients on therapeutic apixaban need surgery:
- Stop 48 hours before high bleeding risk procedures 1
- Stop 24 hours before low bleeding risk procedures 1
- Resume prophylactic dosing (2.5 mg twice daily) can begin 12 hours after last prophylactic LMWH dose once hemostasis achieved 10
Key Clinical Pearls
- The 2.5 mg twice daily dose is specifically for prophylaxis, not treatment
- This dose has demonstrated better bleeding profiles than therapeutic dosing while maintaining efficacy for prevention 5, 6
- In cancer patients completing initial VTE treatment, reduced-dose apixaban (2.5 mg twice daily) after 6 months shows 25% reduction in bleeding versus continuing full-dose 5, 6
- Apixaban prophylaxis does not require routine coagulation monitoring 2
