1:
- Starting dose: 1.5 mg once daily
- Day 15: May increase to 3 mg based on response
- Maximum: 3 mg daily
- Important: Titration intervals <14 days result in higher adverse reaction rates 1
Administration Instructions
- Once daily dosing with or without food 1
- No food restrictions simplify administration
- Available capsule strengths: 0.5 mg, 0.75 mg, 1.5 mg, 3 mg, 4.5 mg, and 6 mg 1
Critical Pharmacokinetic Considerations
Vraylar has an exceptionally long half-life due to its active metabolites (didesmethyl-cariprazine has a 1-3 week half-life), meaning dose changes take several weeks to reach steady state 1, 2, 3. This requires:
- Monitor patients for adverse reactions and treatment response for several weeks after starting or changing doses 1
- Metabolite accumulation occurs over 4-8 weeks 3
- This distinguishes cariprazine from other antipsychotics and demands patience in dose titration
Drug Interactions Requiring Dose Adjustment
Strong CYP3A4 Inhibitors (e.g., ketoconazole, itraconazole, clarithromycin)
When initiating Vraylar 1:
- Schizophrenia/Bipolar Mania/Bipolar Depression/MDD: Start 0.5 mg daily; increase to 0.75 mg if needed
When adding inhibitor to stable Vraylar dose 1:
- If on 1.5-3 mg: Reduce to 0.5 mg daily
- If on 4.5-6 mg: Reduce to 0.75 mg daily
Moderate CYP3A4 Inhibitors (e.g., erythromycin, diltiazem, fluconazole)
When initiating Vraylar 1:
- Start 0.75 mg daily; increase to 1.5 mg if needed
When adding inhibitor to stable Vraylar dose 1:
- If on 1.5-3 mg: Reduce to 0.75 mg daily
- If on 4.5-6 mg: Reduce to 1.5 mg daily
CYP3A4 Inducers
Concomitant use is NOT recommended 1 - avoid combining with rifampin, carbamazepine, phenytoin, St. John's Wort
Pediatric patients should NOT initiate Vraylar while taking strong or moderate CYP3A4 inhibitors 1
Boxed Warnings
1. Increased Mortality in Elderly with Dementia-Related Psychosis
Vraylar is NOT approved for dementia-related psychosis - elderly patients with dementia treated with antipsychotics have increased death risk 1
2. Suicidal Thoughts and Behaviors
Antidepressants (relevant for MDD indication) increase suicide risk in pediatric and young adult patients - closely monitor all patients for clinical worsening and emergence of suicidal ideation 1
Major Safety Precautions
Cerebrovascular Events
Increased stroke and TIA risk in elderly patients with dementia-related psychosis 1
Neuroleptic Malignant Syndrome (NMS)
Potentially fatal - immediately discontinue if suspected and provide intensive symptomatic treatment 1
Tardive Dyskinesia
Risk increases with duration of treatment and cumulative dose - consider discontinuation if signs develop 1
Metabolic Changes 1
Monitor for:
- Hyperglycemia/diabetes mellitus: Check fasting glucose and HbA1c
- Dyslipidemia: Monitor lipid panel
- Weight gain: Regular weight monitoring (though cariprazine shows relatively neutral metabolic profile compared to other antipsychotics 2, 4)
Hematologic Monitoring
In patients with pre-existing low WBC or history of drug-induced leukopenia/neutropenia, perform complete blood counts frequently during first months of therapy 1
Orthostatic Hypotension and Syncope
Monitor heart rate and blood pressure, especially in patients with cardiovascular/cerebrovascular disease or conditions predisposing to hypotension 1
Seizures
Use cautiously in patients with seizure history or conditions lowering seizure threshold 1
Cognitive and Motor Impairment
Caution patients about operating machinery or driving until they know how Vraylar affects them 1
Most Common Adverse Reactions
Based on clinical trials, adverse reactions occurring in ≥5% and at least twice placebo rate 1, 4:
Schizophrenia: Extrapyramidal symptoms, akathisia
Bipolar Mania: Extrapyramidal symptoms, akathisia, dyspepsia, vomiting, somnolence, restlessness
Bipolar Depression: Nausea, akathisia, restlessness, extrapyramidal symptoms
MDD (Adjunctive): Akathisia, restlessness, fatigue, constipation, nausea, insomnia, increased appetite, dizziness, extrapyramidal symptoms
Key tolerability finding: Akathisia and extrapyramidal symptoms are dose-dependent, occurring in 9-19% depending on dose versus 4-8% with placebo 2, 4. These are generally mild-to-moderate and rarely cause discontinuation (4.3% overall) 4.
Special Populations
Pregnancy
Based on animal data, may cause fetal harm - use only if benefit justifies potential fetal risk 1
Lactation
Recent data shows cariprazine and metabolites are detectable in breast milk with cumulative relative infant dose of 2.54% (below standard 10% safety threshold), but two cases of infant lethargy were reported, one resolving with maternal dose reduction 5. The long half-life of didesmethyl-cariprazine raises concerns about metabolite accumulation in infants, particularly with higher maternal doses.
Hepatic/Renal Impairment
Do not use in severe hepatic or renal disease 6
Contraception
Females of childbearing age must use effective contraception during treatment 7
Clinical Pearls
- Unique D3-preferring pharmacology: Cariprazine has 10-fold higher affinity for D3 versus D2 receptors, distinguishing it from other antipsychotics 8, 9, 2
- Efficacy in negative symptoms: Shows superiority over risperidone for predominant negative symptoms in schizophrenia 2, 10
- Metabolically favorable: Weight gain typically <1 kg, neutral effects on glucose/lipids, no prolactin elevation, no QT prolongation 2, 4
- High completion rates: 90% study completion in MDD trials indicates good tolerability 4
- Patience required: Due to long half-life, allow several weeks between dose adjustments and when assessing efficacy 1, 3
