Bimzelx (Bimekizumab) 160 mg/mL Dosing and Administration
For plaque psoriasis, administer 320 mg (given as two 160 mg injections or one 320 mg/2 mL injection) subcutaneously at weeks 0,4,8,12,16, and then every 8 weeks thereafter. 1
Dosing by Indication
Plaque Psoriasis
- Loading phase: 320 mg at weeks 0,4,8,12, and 16
- Maintenance phase: 320 mg every 8 weeks starting at week 16
- Route: Subcutaneous injection
- Can be administered as either two 160 mg/mL injections or one 320 mg/2 mL injection 1
Psoriatic Arthritis (PsA)
- Same dosing regimen as plaque psoriasis: 320 mg at weeks 0,4,8,12,16, then every 8 weeks
- Bimekizumab demonstrated superior ACR50 response (44% vs 10% placebo at week 16) in biologic-naïve patients 2
- Also effective in TNF-inhibitor inadequate responders (43% ACR50 vs 7% placebo) 3
Axial Spondyloarthritis (Ankylosing Spondylitis and Non-radiographic axSpA)
- Dosing: 160 mg every 4 weeks (not 320 mg as in psoriasis/PsA)
- No loading phase required
- Achieved ASAS40 response in 44.8% (radiographic) and 47.7% (non-radiographic) vs ~22% placebo at week 16 4
Hidradenitis Suppurativa
- Dosing: 320 mg at weeks 0,2,4,6,8,10,12, then every 4 weeks thereafter
- Different loading schedule than psoriasis 1
Administration Details
Self-administration is standard - patients can inject subcutaneously at home after proper training 1
Injection sites:
- Rotate between thighs, abdomen (avoiding 2 inches around navel), or outer upper arms
- Do not inject into areas where skin is tender, bruised, red, scaly, or hard 1
Storage:
- Refrigerate at 2-8°C (36-46°F)
- Allow to reach room temperature (30 minutes) before injection
- Protect from light 1
Key Clinical Considerations
Mechanism: Bimekizumab uniquely inhibits both IL-17A and IL-17F, distinguishing it from other IL-17 inhibitors that target only IL-17A 1, 2, 3
Efficacy timeline:
- Significant improvements typically seen by week 4-16
- Peak responses often achieved by week 16-24
- Sustained efficacy demonstrated through 3 years of treatment 5, 6
Common adverse events (>3%):
- Nasopharyngitis and upper respiratory tract infections
- Oral candidiasis and other localized fungal infections (most common distinguishing feature)
- Pharyngitis, diarrhea, headache 4, 5
Important safety monitoring:
- Screen for tuberculosis before initiation
- Monitor for fungal infections (particularly oral candidiasis - typically mild and manageable)
- Low incidence of inflammatory bowel disease, uveitis, and MACE 4, 5
- Avoid live vaccines during treatment 1
Dosing Adjustments
No dose adjustment needed for:
- Age ≥65 years (limited data but no observed differences) 1
- Renal or hepatic impairment (not formally studied but expected minimal impact given IgG1 metabolism) 1
Missed doses: If a dose is missed, administer as soon as possible and resume the regular dosing schedule 1
The dosing differs significantly by indication - psoriasis/PsA uses 320 mg with an 8-week maintenance interval after loading, while axial spondyloarthritis uses 160 mg every 4 weeks without loading, and hidradenitis suppurativa has its own unique schedule. This distinction is critical for proper prescribing.
