Differential Diagnosis for Pressure but not Pain in the Head with Weakness
- Single most likely diagnosis
- Migraine: Often presents with pressure or tightness in the head without pain, and can be accompanied by weakness, especially if the migraine is complicated or has an aura phase.
- Other Likely diagnoses
- Tension Headache: Characterized by a feeling of pressure or tightness around the head, often without sharp pain, and can be associated with general weakness due to stress or fatigue.
- Hypertension: Uncontrolled high blood pressure can cause a sensation of pressure in the head, and if severe, can lead to symptoms like weakness due to potential end-organ damage.
- Anemia: Can cause weakness and sometimes a sensation of pressure in the head due to decreased oxygen delivery to tissues.
- Do Not Miss (ddxs that may not be likely, but would be deadly if missed.)
- Subarachnoid Hemorrhage: Although typically presents with a severe headache, some cases may initially have a pressure sensation without severe pain, and weakness can occur due to increased intracranial pressure or cerebral vasospasm.
- Brain Tumor: Can cause increased intracranial pressure leading to a feeling of pressure in the head and weakness due to local mass effect or increased pressure.
- Stroke: Especially if presenting with atypical symptoms, a stroke can cause weakness and sometimes a sensation of pressure in the head without severe pain.
- Rare diagnoses
- Pseudotumor Cerebri (Idiopathic Intracranial Hypertension): Characterized by increased intracranial pressure without a mass lesion, can cause a sensation of pressure in the head and sometimes weakness.
- Chiari Malformation: A structural defect in the cerebellum that can cause increased intracranial pressure and lead to symptoms like pressure in the head and weakness, especially with certain postures or activities.
- Mitochondrial Myopathies: Rare genetic disorders affecting muscle and nerve function, can cause weakness and sometimes unusual headache presentations, including pressure without pain.
