GFR Thresholds for CKD Staging
Chronic kidney disease is staged based on GFR values, with CKD defined as GFR <60 mL/min/1.73 m² and/or evidence of kidney damage persisting for ≥3 months. 1
CKD Stage Classification by GFR
The K/DOQI and KDIGO guidelines establish five stages of CKD based on GFR thresholds 1, 2:
- Stage 1: GFR ≥90 mL/min/1.73 m² with evidence of kidney damage (proteinuria, imaging abnormalities, or other markers)
- Stage 2: GFR 60-89 mL/min/1.73 m² with evidence of kidney damage
- Stage 3a: GFR 45-59 mL/min/1.73 m²
- Stage 3b: GFR 30-44 mL/min/1.73 m²
- Stage 4: GFR 15-29 mL/min/1.73 m²
- Stage 5: GFR <15 mL/min/1.73 m² (kidney failure)
Critical GFR Threshold
The GFR threshold of <60 mL/min/1.73 m² is clinically significant because it represents loss of half or more of normal adult kidney function. Below this level, the prevalence of CKD complications (anemia, bone disease, malnutrition, cardiovascular disease) increases substantially 1. This threshold applies regardless of age, despite age-related GFR decline, because decreased GFR in elderly patients remains an independent predictor of death and cardiovascular disease 1.
Stage 3 Subdivision Rationale
The subdivision of Stage 3 into 3a and 3b was driven by data showing different risk profiles and outcomes between these GFR ranges 2. Patients with GFR 30-44 mL/min/1.73 m² (Stage 3b) have significantly higher risks for progression to kidney failure and cardiovascular events compared to those with GFR 45-59 mL/min/1.73 m² (Stage 3a).
GFR Estimation Methods
Use creatinine-based eGFR (eGFRcr) for initial assessment, with combined creatinine-cystatin C equations (eGFRcr-cys) preferred when cystatin C is available 3, 2. The CKD-EPI equation is recommended over the MDRD equation for adults 2. Recent evidence confirms that eGFRcr-cys most accurately represents measured GFR associations with mortality, while eGFRcr underestimates and eGFRcys overestimates mortality risk 4.
Clinical Action Points by Stage
- Stages 1-2: Initiate blood pressure control, treat underlying causes, reduce cardiovascular risk 1
- Stage 3: Begin evaluation and treatment of complications (anemia, bone disease, malnutrition) 1
- Stage 4 (GFR <30 mL/min/1.73 m²): Refer to nephrology for co-management and preparation for kidney replacement therapy 1
- Stage 5 (GFR <15 mL/min/1.73 m²): Prepare for dialysis or transplantation 1
Important Caveats
A single abnormal GFR or albuminuria measurement does not confirm CKD—chronicity requires documentation over ≥3 months 3. However, if CKD is clinically likely based on other indicators (imaging showing small kidneys, known diabetic nephropathy), treatment can begin at first presentation without waiting 3 months 3.
The definition applies uniformly across all ages, though approximately 17% of persons >60 years have eGFR <60 mL/min/1.73 m² 1. This age-related prevalence does not negate the clinical significance, as reduced GFR requires medication dose adjustments and predicts adverse outcomes at any age 1.
