How to Administer Tetanus Toxoid 1 cc IM Using a 1 cc Syringe

Administer 0.5 mL (not 1 cc) of tetanus toxoid intramuscularly into the deltoid muscle. The standard dose for all tetanus-containing vaccines in adults is 0.5 mL, not 1 cc 1.

Critical Dosing Clarification

  • Standard dose = 0.5 mL for all tetanus toxoid-containing vaccines (Td, Tdap) 1
  • A 1 cc syringe is appropriate for drawing up and administering this 0.5 mL dose
  • Do not administer a full 1 cc (1 mL) - this would be double the approved dose

Injection Technique

Site Selection

Inject 4 cm distal to the posterolateral corner of the acromion - this is more posterior and inferior than many clinicians traditionally inject 2. This site:

  • Maximizes successful intramuscular delivery
  • Minimizes risk of overpenetration
  • Avoids axillary nerve injury

Needle Length Selection

Choose needle length based on patient weight:

  • <60 kg: 0.625" (5/8 inch) needle
  • 60-90 kg (females) or 60-118 kg (males): 1" needle
  • >90 kg (females) or >118 kg (males): 1.5" needle 2

Administration Steps

  1. Draw up exactly 0.5 mL of vaccine into your 1 cc syringe
  2. Do not change the needle after drawing up the vaccine - this does not reduce adverse reactions and may actually increase them 3
  3. Do not wipe the needle with gauze - this practice increases febrile and systemic reactions 3
  4. Insert needle at 90-degree angle into deltoid muscle at the site described above
  5. Aspirate is not required per current practice
  6. Inject the full 0.5 mL dose
  7. Withdraw needle and apply gentle pressure

Important Caveats

If administering Tetanus Immune Globulin (TIG) simultaneously (for wound management in unvaccinated patients), use separate syringes and inject at different anatomical sites 4.

Avoid more frequent dosing than recommended - administering tetanus toxoid more often than every 5-10 years (depending on indication) significantly increases adverse reactions including Arthus-type hypersensitivity 1.

Have syncope precautions in place - fainting has been reported following tetanus vaccination, so have the patient sit or lie down and observe for 15 minutes post-injection 1.
