Clonidine will not effectively treat opioid cravings in opioid withdrawal. The treatment of opioid withdrawal often involves the use of α2-adrenergic agonists such as clonidine or lofexidine, as well as other medications targeting specific withdrawal symptoms 1. Clonidine is effective for many withdrawal symptoms like anxiety, agitation, sweating, runny nose, tearing, muscle aches, and hypertension due to its action on alpha-2 adrenergic receptors in the central nervous system. However, opioid cravings are a complex symptom that may not be fully addressed by clonidine alone. According to a systematic review by Gowing et al 1, buprenorphine is more effective than clonidine or lofexidine for managing opioid withdrawal in terms of severity of withdrawal, duration of withdrawal treatment, and the likelihood of treatment completion. This suggests that while clonidine can help with some symptoms of opioid withdrawal, it may not be sufficient for treating opioid cravings, which are a key component of the withdrawal syndrome. Some studies have shown that buprenorphine is probably similar in effectiveness to tapered doses of methadone in the treatment of opioid withdrawal, and both are more effective than clonidine for this purpose 1. Therefore, for comprehensive management of opioid withdrawal, including cravings, medications like buprenorphine or methadone may be more effective than clonidine alone.
