From the Guidelines
If you develop a rash on your back while taking Eliquis (apixaban), contact your healthcare provider immediately as this could be a sign of an allergic reaction. When you call your provider, describe the appearance of the rash, when it started, any other symptoms you're experiencing (such as itching, swelling, difficulty breathing, or fever), and any new medications or products you've recently used. While the provided evidence 1 discusses the management of cutaneous side effects in patients with chronic myeloid leukemia, the general principle of managing a rash as a potential adverse event can be applied to this situation. According to the evidence 1, mild to moderate adverse events can be managed with topical therapies or systemic therapies with antihistamines or short courses of systemic steroids. However, it's crucial to consult your healthcare provider before taking any action, as they will determine the best course of treatment based on the severity of the rash and your individual situation. Do not stop taking Eliquis without medical advice, as suddenly discontinuing the medication can increase your risk of blood clots and stroke. Take photos of the rash if possible to show your doctor, and while waiting for medical advice, avoid scratching the affected area, wear loose cotton clothing, and don't apply any creams or medications to the rash unless directed by your provider. If you experience severe symptoms like difficulty breathing, facial swelling, or feel faint, seek emergency medical attention immediately as these could indicate a serious allergic reaction. In general, the management of a rash caused by a medication like Eliquis would involve a thorough evaluation by a healthcare provider to determine the cause and appropriate treatment, which may include topical or systemic therapies, or in severe cases, discontinuation of the medication.

From the Research
Eliquis (Apixaban) and Rash
- Eliquis (apixaban) is a medication used to prevent blood clots and stroke in patients with atrial fibrillation [ 3 ].
- There have been reports of rash associated with the use of apixaban, although it is considered a rare side effect [ 4 , 5 ].
- A case report published in 2023 described a 74-year-old female patient who developed a pruritic coalescent erythematous dermatitis throughout her torso, back, and lower extremities three days after starting apixaban [ 4 ].
- Another case report published in 2021 described a 62-year-old man who developed a hemorrhagic pruritic rash around his buttocks area and extending into his groin after receiving the first dose of apixaban [ 5 ].
Management of Rash
- If a rash develops while taking Eliquis, it is essential to consult a healthcare provider for proper evaluation and management [ 4 , 5 ].
- In some cases, the rash may resolve on its own after discontinuing the medication [ 4 ].
- Alternative anticoagulant medications, such as dabigatran or warfarin, may be considered if the rash is severe or persistent [ 4 , 5 ].
Comparison with Other Anticoagulants
- A study published in 2021 compared the effectiveness of rivaroxaban and apixaban in patients with atrial fibrillation and found that apixaban was associated with a lower risk of major ischemic or hemorrhagic events [ 3 ].
- However, the study did not specifically address the risk of rash associated with these medications [ 3 ].
