From the Guidelines

Antibiotic Treatment Duration for Hinchey 1B Diverticulitis

The antibiotic treatment duration for Hinchey 1B diverticulitis is not necessary in most cases, as observational treatment without antibiotics can be considered appropriate for patients with a first episode of CT-proven acute left-sided colonic diverticulitis (ALCD) Hinchey stages 1a and 1b 1.

  • However, if antibiotic therapy is necessary, the treatment typically consists of a 10-day course of antibiotics, such as IV amoxicillin-clavulanic acid, 1200 mg four times daily for at least 48 h, followed by oral administration of 625 mg three times daily 1.
  • The choice of antibiotic therapy should cover Gram-negative bacilli and anaerobes, and oral administration may be equally as effective as intravenous administration 1.
  • It is essential to note that the decision to use antibiotics should be based on the individual patient's risk factors, such as immunocompromised status, elderly age, or comorbidities, and the presence of systemic manifestations of infection 1.
  • In general, antibiotics can be safely omitted in patients with a first episode of uncomplicated (Hinchey 1a) ALCD, and similar results were found for Hinchey 1b diverticulitis, although the evidence is limited 1.

From the Research

Antibiotic Treatment Duration for Hinchey 1B Diverticulitis

  • The provided studies do not specifically address the antibiotic treatment duration for Hinchey 1B diverticulitis 2, 3, 4, 5, 6.
  • However, a study on complicated diverticulitis suggests that extending antibiotic treatment beyond 10 days may not be necessary 6.
  • Another study mentions that there is a paucity of data regarding the optimal duration of antimicrobial therapy for acute diverticulitis 4.
  • The general recommendation is to use antibiotics with activity against common gram-negative and anaerobic pathogens, but there is little evidence to base selection of specific antimicrobial regimens 4.
  • Some studies suggest that antibiotic treatment may not be necessary for uncomplicated diverticulitis, and that a clear-liquid diet along with bowel rest may be sufficient 3, 5.
