Excessive estradiol in hormone replacement therapy (HRT) can cause several side effects that should prompt dosage adjustment, including breast tenderness, bloating, nausea, headaches, mood swings, fluid retention, and breakthrough bleeding, as reported in studies such as 1 and 1.

Recommended Dosage Adjustments
For transdermal estradiol, typical doses range from 0.025-0.1 mg/day patches, while oral estradiol is usually prescribed at 0.5-2 mg daily, as noted in 1 and 1. Your provider may recommend lowering your dose gradually rather than stopping abruptly to avoid withdrawal symptoms. Blood tests can help determine if your estradiol levels are too high (optimal range is typically 50-200 pg/mL for most postmenopausal women on HRT), as discussed in 1 and 1. These side effects occur because estrogen receptors throughout the body become overstimulated when levels are too high, affecting tissues in the breasts, brain, and reproductive organs. Finding the right balance is important, as too little estradiol won't provide symptom relief while too much increases health risks including blood clots and certain cancers, as highlighted in 1, 1, and 1.

- The presence of other predisposing factors, such as increasing age, obesity, and thrombophilia 4
- The duration of use, with the risk of venous thrombosis being greatest during the first year of use 4
