As an antihypertensive, clonidine works by stimulating alpha-2 adrenergic receptors in the brain, which reduces sympathetic outflow and lowers blood pressure 1. Beyond hypertension, clonidine is commonly prescribed for ADHD in children and adults when stimulants aren't appropriate, as it enhances noradrenergic neurotransmission in the prefrontal cortex, strengthening the regulatory role of the prefrontal cortex 1. It's also effective for managing withdrawal symptoms from opioids, alcohol, and nicotine, as well as menopausal hot flashes, Tourette syndrome, anxiety disorders, and certain pain conditions, particularly neuropathic pain. For hypertension, typical dosing starts at 0.1 mg twice daily, potentially increasing to 0.2-0.3 mg twice daily, with a maximum of 2.4 mg daily 1.
