From the Guidelines
To effectively use ofloxacin otic solution for treating ear infections, lie down with the affected ear up, fill the ear canal with the prescribed number of drops, and remain in this position for 3 to 5 minutes to allow the medication to penetrate the ear canal fully. When administering the drops, it is recommended to have someone assist you if possible, and to use a gentle to-and-fro movement of the ear or tragal pumping to help the drops reach their intended destination 1. The ear canal should be cleared of any debris or obstruction before administering the drops, and it is generally advised to keep the ear dry while using the ear drops 1. It is also important to note that if the drops do not easily run into the ear canal, you may need to have the ear canal cleaned by your clinician or have a wick placed in the ear canal to help the drops penetrate 1.
Some key points to consider when using ofloxacin otic solution include:
- Having someone assist you in administering the drops if possible
- Lying down with the affected ear up and filling the ear canal with the prescribed number of drops
- Remaining in this position for 3 to 5 minutes to allow the medication to penetrate the ear canal fully
- Using a gentle to-and-fro movement of the ear or tragal pumping to help the drops reach their intended destination
- Keeping the ear dry while using the ear drops
- Avoiding touching the dropper tip to prevent contamination
- Completing the full course of treatment even if symptoms improve before finishing, as ofloxacin works by killing the bacteria causing the infection through inhibition of bacterial DNA gyrase, preventing bacterial reproduction 1.
It is essential to follow these directions carefully and consult your healthcare provider if you have any questions or concerns, or if your symptoms worsen or do not improve after a few days of treatment.

How should ofloxacin otic solution be given?
- Wash hands
- Clean ear & warm bottle
- Add drops For a Middle Ear Infection: The person receiving ofloxacin otic solution should lie on his/her side with the infected ear up. Patients (12 and older) should have 10 drops of ofloxacin otic solution put into the infected ear Pediatric patients under 12 should have 5 drops put into the infected ear. For an Ear Canal Infection (“Swimmer’s Ear”): The person receiving ofloxacin otic solution should lie on his/her side with the infected ear up Patients (13 and older) should have 10 drops of ofloxacin otic solution put into the infected ear. Pediatric patients under 13 should have 5 drops put into the infected ear. 4 Press ear or pull ear For a Middle Ear Infection: While the person receiving ofloxacin otic solution lies on his/her side, the person giving the drops should gently press the TRAGUS (see diagram) 4 times in a pumping motion. For an Ear Canal Infection (“Swimmer’s Ear”): While the person receiving the drops lies on his/her side, the person giving the drops should gently pull the outer ear upward and backward.
- Stay on side The person who received the ear drops should remain on his/her side for at least 5 minutes
The directions for using ofloxacin otic solution are as follows:

From the Research
Directions for Using Ofloxacin Otic Solution
- The typical dosage of ofloxacin otic solution is 0.3% solution, with the number of drops depending on the patient's age 3, 4, 5.
- For children aged 6 months to <13 years, 5 drops are instilled in the affected ear once daily, while adolescents and adults aged > or = 13 years receive 10 drops once daily 4.
- The treatment duration is usually 7 days, with assessments conducted at the end-of-treatment visit and 7 to 10 days later (the test-of-cure visit) 4, 5.
- Ofloxacin otic solution is effective in treating otitis externa, chronic suppurative otitis media, and otitis media in patients with tympanic membrane perforations or tympanostomy tubes 3, 6.
- The solution has a broad spectrum of activity, covering most common ear pathogens, and is well tolerated with minimal adverse effects 3, 6.
- The lack of ototoxicity associated with ofloxacin otic solution makes it a safe option for use in patients with undiagnosed tympanic membrane perforation 3, 6, 7.
Administration and Safety
- Ofloxacin otic solution should be instilled in the affected ear, with the patient's head tilted to allow the solution to flow into the ear canal 4.
- The solution is not associated with ototoxicity, and no increase in bone-conduction threshold has been shown after treatment 6.
- Adverse reactions to ofloxacin otic solution are generally mild, with the most common adverse events being bitter taste, pruritus, and application-site reactions 3, 4, 6.
Efficacy and Comparison to Other Treatments
- Ofloxacin otic solution has been shown to be as effective as other otic preparations, including neomycin sulfate/polymyxin B sulfate/hydrocortisone otic suspension, in treating otitis externa 5, 7.
- The solution has a high cure rate, with 91% of patients achieving clinical cure in one study 4.
- Ofloxacin otic solution is also effective in eradicating Pseudomonas aeruginosa, a common pathogen in ear infections, with eradication rates of 96% to 100% 3, 6.
