The recommended dose of Jardiance (Empagliflozin) for adults with type 2 diabetes is 10 mg once daily. This dose is based on the most recent and highest quality study, which recommends initiating SGLT2 inhibitors with CV benefit at the lowest dose tested in CV and renal outcomes trials 1. The study notes that no further dose titration is needed for CV or renal risk reduction, but dose increases may provide further glucose reduction benefits if indicated.
Key Considerations
- The medication should be taken in the morning with or without food.
- Patients with kidney impairment may require dose adjustments; specifically, Jardiance should not be initiated if eGFR is below 45 mL/min/1.73 m² for glycemic control, as stated in the 2020 expert consensus decision pathway 1.
- However, a more recent study from 2022 suggests that the use of Empagliflozin is not recommended with eGFR <45 mL/min/1.73 m² 1.
- The medication works by inhibiting sodium-glucose cotransporter-2 (SGLT2) in the kidneys, which prevents glucose reabsorption and increases glucose excretion in urine, thereby lowering blood glucose levels.
- Patients should be advised to stay adequately hydrated while taking Jardiance and to monitor for side effects such as urinary tract infections, genital mycotic infections, and hypoglycemia, especially if they are also taking insulin or insulin secretagogues.
- Regular kidney function monitoring is recommended during treatment.
Important Side Effects and Interactions
- Euglycemic diabetic ketoacidosis is a potential side effect of Empagliflozin, as noted in the study 1.
- The medication may contribute to intravascular volume contraction, and patients should be cautioned about this potential effect 1.
- Patients with prior amputation, severe peripheral neuropathy, severe peripheral vascular disease, or active diabetic foot ulcers or soft tissue infections should use Empagliflozin with caution 1.
