Next Steps for Elevated CA19-9 Level
For a patient with an elevated CA19-9 level, the next step should be comprehensive imaging with either MDCT (multidetector computed tomography) or MRI/MRCP (magnetic resonance imaging/magnetic resonance cholangiopancreatography) to evaluate for possible pancreatic, biliary, or other gastrointestinal malignancies. 1
Understanding CA19-9 Elevation
CA19-9 is a tumor marker commonly associated with several conditions:
- Malignant conditions: Pancreatic cancer, cholangiocarcinoma, gastric cancer, colorectal cancer, and other gastrointestinal malignancies 1, 2
- Benign conditions: Biliary obstruction, cholangitis, cholelithiasis, pancreatitis, liver cirrhosis, and other non-malignant conditions 2, 3

- Extremely high levels are more suggestive of malignancy, though benign conditions can occasionally cause marked elevations 2, 3
