Ear drops containing antibiotics, such as ciprofloxacin (Ciprodex), ofloxacin, or neomycin-polymyxin B combinations, are effective for treating ear infections, particularly those affecting the outer ear canal, and should be used for at least 7 days, even if symptoms improve sooner, to prevent relapse of infection 1.

- Patients with a nonintact tympanic membrane require special consideration, and the choice of ototopical agent may need to be adjusted accordingly 1.
- It is essential to keep the ear dry during treatment and to complete the full course of medication, even if symptoms improve quickly, to ensure effective treatment and prevent complications 1.
Treatment Duration and Administration
- Treatment with ear drops should last for at least 7 days, with drops applied 2-4 times daily as directed 1.
- If symptoms persist beyond 7 days, the patient should notify their doctor and continue the drops until the symptoms resolve, for a maximum of 7 additional days 1.
Pain Relief and Potential Complications
- For pain relief while waiting for antibiotics to work, over-the-counter analgesic ear drops containing benzocaine or similar ingredients can provide temporary comfort, but pain-relieving (anesthetic) eardrops are not recommended during an active ear canal infection 1.
- If symptoms do not improve within 48-72 hours, or if there is severe pain, fever, or discharge, or if the eardrum is perforated, medical attention should be sought as different treatment may be needed 1.

The recommended dosage regimen for the treatment of otitis externa is: For pediatric patients (from 6 months to 13 years old): Five drops (0.25 mL, 0.75 mg ofloxacin) instilled into the affected ear once daily for seven days. For patients 13 years and older: Ten drops (0.5 mL, 1. 5 mg ofloxacin) instilled into the affected ear once daily for seven days. The recommended dosage regimen for the treatment of acute otitis media in pediatric patients (from 1 to 12 years old) with tympanostomy tubes is: Five drops (0.25 mL, 0. 75 mg ofloxacin) instilled into the affected ear twice daily for ten days. The recommended dosage regimen for the treatment of chronic suppurative otitis media with perforated tympanic membranes in patients 12 years and older is: Ten drops (0.5 mL, 1. 5 mg ofloxacin) instilled into the affected ear twice daily for fourteen days.
