Chemotherapy-Induced Nausea and Vomiting
Highly Emetogenic Chemotherapy
- Single dose regimen: 24 mg oral dose administered 30 minutes before chemotherapy 1
- This single 24 mg dose has been shown to be more effective than 8 mg twice daily regimen for highly emetogenic chemotherapy 1
Moderately Emetogenic Chemotherapy
- Standard regimen: 8 mg oral twice daily 2
- First dose given 30 minutes before chemotherapy, second dose 8 hours later
- Continue 8 mg twice daily for 1-2 days after completion of chemotherapy
Radiation-Induced Nausea and Vomiting
- Standard dose: 8 mg oral three times daily 2
- For breakthrough therapy, can titrate up as needed to maximum of 16 mg oral or IV daily 2
Postoperative Nausea and Vomiting
- Single dose regimen: 16 mg as a single oral dose one hour before anesthesia induction 1
Administration Forms
Ondansetron is available in multiple formulations:
- Oral tablets: 8 mg, 24 mg
- Oral dissolving tablets: 8 mg
- Oral soluble film: 8 mg
- IV solution: 8 mg or 0.15 mg/kg 2

Important Considerations
Safety Concerns
- QT prolongation risk: Use with caution in patients taking other medications that can prolong QT interval 2

Practical Application
For most adult patients requiring antiemetic therapy:
- Start with 8 mg for routine prevention of nausea/vomiting
- For highly emetogenic chemotherapy, use the 24 mg single dose approach
- For breakthrough nausea/vomiting, additional doses can be given up to the maximum daily limit of 32 mg
- Space doses at least 4 hours apart when giving multiple doses
