Tetanus Prophylaxis for a 2-Year-Old with a Metal Cut
For a 2-year-old child with a cut from metal at home, DTaP (diphtheria, tetanus toxoids, and acellular pertussis vaccine) should be administered if the child has received fewer than 3 doses or has an unknown vaccination history, along with tetanus immune globulin (TIG) if the wound is contaminated. 1
Assessment of Vaccination Status and Wound Type
First, determine the child's tetanus vaccination status:
- Has the child completed the primary 3-dose DTaP series?
- When was the last tetanus-containing vaccine administered?
Next, assess the wound characteristics:
- Is this a clean, minor wound or a contaminated/tetanus-prone wound?
- Metal protruding from carpet may have dirt or other contaminants, potentially classifying it as tetanus-prone
Tetanus Prophylaxis Recommendations
Based on Vaccination History:
If vaccination history is unknown or <3 doses received:
- Administer DTaP (age-appropriate for a 2-year-old)
- For tetanus-prone wounds, also administer TIG (250 units IM)
- Complete the primary vaccination series at subsequent visits
If ≥3 doses received:
- No DTaP needed if last dose was <5 years ago
- For tetanus-prone wounds, administer DTaP if last dose was ≥5 years ago
- For clean, minor wounds, administer DTaP if last dose was ≥10 years ago
Administration Details:
- If both DTaP and TIG are indicated, administer at different sites using separate syringes 1
- The recommended prophylactic dose of TIG is 250 units IM for children and adults 1, 2
Special Considerations
- Children are more likely to be up-to-date with vaccinations compared to adults, with >96% of children aged 19-35 months having received 3 doses of tetanus-containing vaccines 1
- If the child has an unknown vaccination history, treat as if they have had no previous tetanus toxoid doses 1
- The wound from metal protruding from carpet should be thoroughly cleaned and debrided as proper wound care is critical for tetanus prevention 1
Common Pitfalls to Avoid
Delaying prophylaxis: Tetanus can be fatal, with mortality rates of 18% reported in the US (1998-2000) 1. Prompt prophylaxis is essential.
Misclassifying the wound: Metal protruding from carpet at home may have dirt or other contaminants, potentially making it a tetanus-prone wound requiring more aggressive prophylaxis.
Assuming vaccination status: Never assume vaccination status without documentation. If uncertain, treat as if the child has had no previous tetanus vaccination 1, 2.
Using inappropriate vaccine: For children <7 years old, DTaP is the appropriate tetanus-containing vaccine, not Td or Tdap which are for older children and adults 1.
Failing to complete the primary series: Ensure the child completes the full primary vaccination series at subsequent visits if they haven't already done so 1.
