Mechanism of Action of Memantine
Memantine works primarily as a low to moderate affinity uncompetitive (open-channel) NMDA receptor antagonist that blocks excessive glutamate activity in the brain, which is believed to contribute to the symptoms of Alzheimer's disease. 1
Primary Mechanism
Memantine acts through the following mechanisms:
NMDA Receptor Antagonism: Memantine partially blocks the N-methyl-D-aspartate (NMDA) receptor and prevents excess stimulation of the glutamate system 2
Channel Blocking Properties:
- Fast channel-blocking kinetics
- Strong voltage-dependence
- Reduced trapping (allowing exit from closed channels) 3
- These properties allow memantine to block pathological activity while sparing normal function
Pharmacodynamic Properties
Memantine shows additional receptor interactions:
- Low to negligible affinity for GABA, benzodiazepine, dopamine, adrenergic, histamine and glycine receptors 1
- Minimal affinity for voltage-dependent Ca²⁺, Na⁺ or K⁺ channels 1
- Antagonistic effects at the 5HT3 receptor with similar potency to its NMDA effects 1
