The provider should recommend Sumatriptan (Imitrex) or other triptans such as zolmitriptan nasal, sumatriptan nasal, rizatriptan ODT, or almotriptan oral for the treatment of acute migraine attacks. This patient is presenting with classic symptoms of migraine headache, including unilateral pulsatile pain, nausea/vomiting, photophobia (sensitivity to light), phonophobia (sensitivity to sound), and visual aura (flash of light across her eyes) 1. The headaches are recurrent, last several hours, and are associated with sensory sensitivities, which are all characteristic of migraines. Sumatriptan and other triptans are first-line abortive therapies for acute migraine attacks as they work by stimulating serotonin receptors to constrict blood vessels and reduce inflammation in the brain. They are particularly effective when taken early in the course of a migraine.

Treatment Approach
- Counsel the patient to treat acute migraine early in the attack 1.
- Consider using ibuprofen to treat pain in children/adolescents; in adolescents, consider sumatriptan/naproxen oral, zolmitriptan nasal, sumatriptan nasal, rizatriptan ODT, or almotriptan oral 1.
- If one triptan is ineffective, try another or a non-steroidal anti-inflammatory drug-triptan combination 1.
- If nausea/vomiting is present, consider using a non-oral triptan or adding an anti-emetic 1.

The efficacy of sumatriptan tablets in the acute treatment of migraine headaches was demonstrated in 3, randomized, double-blind, placebo-controlled trials. In all 3 trials, the percentage of patients achieving headache response 2 and 4 hours after treatment was significantly greater among patients receiving sumatriptan tablets at all doses compared with those who received placebo There is evidence that doses above 50 mg do not provide a greater effect than 50 mg.
The recommended treatment for a patient presenting with symptoms suggestive of migraine headaches is Sumatriptan 50 mg. This dose has been shown to be effective in achieving headache response and reducing associated symptoms such as nausea and photophobia, with no significant additional benefit from higher doses 2.

Safety and Tolerability
- Sumatriptan is generally well tolerated, with most adverse events being transient and mild 3, 4, 5, 7.
- Common adverse events include nausea, vomiting, malaise, fatigue, and dizziness, as wellinetion site reactions with subcutaneous administration 3, 4, 5, 7.
- Sumatriptan is contraindicated in patients with a history of cardiovascular disease, previous myocardial infarction, Prinzmetal (variant) angina, and uncontrolled hypertension 3, 5.
