Flumazenil Dosing for Benzodiazepine Reversal
For reversal of benzodiazepine effects, the recommended initial dose of flumazenil is 0.2 mg (2 mL) administered intravenously over 15 seconds, with additional doses of 0.2 mg at 60-second intervals up to a maximum total dose of 1 mg. 1
Adult Dosing Protocol
For Conscious Sedation Reversal
- Initial dose: 0.2 mg IV over 15 seconds
- Wait 45 seconds to assess response
- If needed, give additional 0.2 mg doses at 60-second intervals
- Maximum total dose: 1 mg (10 mL)
- Most patients respond to doses between 0.6-1 mg 1
For Resedation
- May repeat doses at 20-minute intervals as needed
- No more than 1 mg should be given at any one time
- No more than 3 mg should be given in any one hour 1
For Benzodiazepine Overdose
- Initial dose: 0.2 mg IV over 15 seconds
- In overdose situations, may titrate up to 3-5 mg total
- Consider slower titration (0.2 mg/min) in overdose cases to minimize emergence reactions 1
Pediatric Dosing Protocol (>1 year of age)
- Initial dose: 0.01 mg/kg (up to 0.2 mg) IV over 15 seconds
- Additional doses: 0.01 mg/kg (up to 0.2 mg) at 60-second intervals
- Maximum total dose: 0.05 mg/kg or 1 mg, whichever is lower 2, 1
Administration Considerations
- Administer through freely running IV into a large vein to minimize injection site pain
- Compatible with 5% dextrose, lactated Ringer's, and normal saline solutions

Contraindications and Cautions
- Not recommended in patients with seizure history or benzodiazepine dependence 2
- Flumazenil has no role in cardiac arrest related to benzodiazepine poisoning 2
- May precipitate withdrawal seizures in benzodiazepine-dependent patients 1
- Contraindicated in tricyclic antidepressant overdose (may induce seizures or arrhythmias) 2
Special Populations

- In mixed overdoses with opioids, administer naloxone first before flumazenil 2
Clinical Pearls
- Duration of action is short (30-60 minutes) and may be shorter than the benzodiazepine being reversed 3
- Monitor patients for at least 2 hours after the last dose of flumazenil 2
- Flumazenil is safe in select low-risk presentations (pediatric exploratory ingestions, iatrogenic overdoses during procedural sedation) 2
- The risks of flumazenil likely exceed the benefit in patients with undifferentiated coma where history and substances involved are unknown 2
