Penlac (Ciclopirox) Dosing for Fungal Infections
For the treatment of onychomycosis, Penlac (ciclopirox 8% nail lacquer) should be applied once daily to affected nails for up to 48 weeks. 1
Recommended Application Protocol
- Formulation: 8% ciclopirox nail lacquer solution
- Frequency: Apply once daily
- Duration:
- Fingernails: Up to 24 weeks

- Apply to the entire nail plate and approximately 5mm of surrounding skin

Patient Selection
Ciclopirox nail lacquer is most appropriate for:
- Mild to moderate onychomycosis
- Distal lateral subungual onychomycosis (DLSO) affecting less than 80% of the nail plate

- Patients who cannot tolerate or have contraindications to oral antifungals
Monitoring and Follow-up
- Evaluate treatment response every 4-12 weeks
- Continue treatment for the full recommended duration even if improvement is seen earlier
- Assess for adverse effects, particularly periungual and nail fold erythema 1
Limitations and Alternative Treatments
For patients with poor response to ciclopirox, consider:
- Newer formulations like ciclopirox 8% hydrolacquer (P-3051), which may provide higher cure rates 3, 4
- Oral antifungals for more severe or resistant cases:
Common Pitfalls to Avoid
- Inadequate treatment duration: Premature discontinuation is common but reduces efficacy
- Insufficient nail preparation: Failure to remove loose nail material and trim affected nails
- Misdiagnosis: Ensure proper diagnosis with microscopy and culture before initiating treatment
- Unrealistic expectations: Inform patients that complete cure rates are modest and treatment requires patience
- Matrix involvement: Topical therapy alone is less effective when the nail matrix is involved 6
