Pain Management for Ruptured Eardrum

For a ruptured eardrum (tympanic membrane perforation), oral acetaminophen or nonsteroidal anti-inflammatory drugs (NSAIDs) are the recommended first-line pain management options, with combination analgesics containing opioids reserved for severe pain. 1

First-Line Pain Management Options

Mild to Moderate Pain

  • Acetaminophen (Paracetamol)

    • Dosage: 500-1000 mg every 4-6 hours
    • Maximum daily dose: 4-6 grams 1
    • Onset of action: 15-30 minutes
    • Caution: Hepatotoxicity
  • NSAIDs (if not contraindicated)

    • Ibuprofen: 400-600 mg every 4-6 hours (maximum 2400 mg/day)
    • Naproxen: 250-500 mg every 8-12 hours (maximum 1500 mg/day)
    • Onset of action: <30 minutes
    • Caution: GI and renal toxicity 1

Severe Pain

  • Combination analgesics (acetaminophen with opioid)
    • Hydrocodone/acetaminophen: 1 tablet every 4-6 hours as needed
    • Use lowest effective dose for shortest duration 2
    • Maximum daily dose: 6 tablets for 7.5 mg/325 mg or 10 mg/325 mg formulations 2

Important Considerations

Topical Anesthetics

  • Do NOT use benzocaine otic solutions or other topical anesthetic ear drops when the tympanic membrane is perforated as they:
    1. Are not FDA-approved for use in the middle ear 1
    2. May mask progression of underlying disease 1
    3. Can potentially cause ototoxicity when they enter the middle ear

Pain Management Principles

  • Pain from a ruptured eardrum can be intense due to the highly sensitive periosteum of the underlying bone 1
  • Early treatment at appropriate starting doses is more effective than treating established pain 1
  • For frequent dosing requirements, fixed interval administration may be more effective than as-needed (prn) dosing 1
  • Ongoing assessment of pain severity is essential for proper management 1

Special Considerations

  • Keep the ear dry by avoiding water entry until cleared by a physician 3
  • Most tympanic membrane perforations from otitis media heal spontaneously within 2-3 weeks 3
  • Re-evaluation within 48-72 hours is recommended if symptoms don't improve 3

When to Seek Further Medical Attention

  • If pain persists despite appropriate analgesic therapy
  • Development of:
    • Vertigo
    • Significant hearing loss
    • Fever
    • Purulent discharge from the ear
    • Worsening symptoms after 48-72 hours

Remember that pain management is an essential component of treating a ruptured eardrum, and adequate analgesia helps patients achieve comfort, rest, and resume normal activities while the perforation heals.
