Methods for Assessing Creatinine Clearance
Calculated Creatinine Clearance (Most Common Method)
- The Cockcroft-Gault equation is the most widely used formula to estimate creatinine clearance:
- CrCl (ml/min) = [(140 - age) × weight (kg)]/[72 × serum creatinine (mg/dL)] × (0.85 if female) 2
- This calculation requires:
- Serum creatinine (from a basic metabolic panel)
- Patient's age
- Patient's weight
- Patient's sex
24-Hour Urine Collection (Gold Standard)
- Requires collection of all urine over a 24-hour period
- Simultaneously measures serum creatinine
- Calculates actual creatinine clearance rather than estimating it

Alternative GFR Estimation Methods
eGFR Equations
- MDRD (Modification of Diet in Renal Disease) formula:
- GFR (ml/min/1.73 m²) = 186 × [SCr (mg/dl)]^-1.154 × [age]^-0.203 × [0.742 if female] × [1.21 if African American] 1
- CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) equation:
