Components of the ALGOPLUS Pain Assessment Scale
The ALGOPLUS scale consists of five behavioral items specifically designed to assess acute pain in older adults who are unable to communicate verbally.
The Five Components of ALGOPLUS
ALGOPLUS is a validated behavioral pain assessment tool that includes the following five components 1:
Facial expressions - Observing for signs such as slight frown, sad or frightened face, grimacing, wrinkled forehead, closed or tightened eyes, distorted expressions, or rapid blinking
Verbal complaints - Listening for vocalizations such as sighing, moaning, groaning, grunting, calling out, or noisy breathing
Look/attention to painful area - Observing if the patient is focusing on or protecting a particular body part
Body position - Noting rigid or tense body posture, guarding, restricted movement, or changes in gait or mobility
Atypical behavior - Identifying changes in interpersonal interactions, activity patterns, or mental status that may indicate pain
Scoring and Interpretation
- Each item is scored as either present (1) or absent (0) using a yes/no format
- The total score ranges from 0 to 5
- A score ≥2 out of 5 is considered the threshold for the presence of acute pain in elderly patients unable to communicate verbally 1
- The assessment takes approximately 1 minute to complete, making it particularly suitable for acute care settings 1
