Clindamycin Preoperative Skin Preparation Dosage
For preoperative skin antisepsis, clindamycin is not recommended as a first-line agent. Instead, alcohol-based chlorhexidine solutions are the preferred agents for preoperative skin preparation. 1
Recommended Preoperative Skin Antisepsis
First-line Options:
- Chlorhexidine with alcohol (preferred option)
Alternative Options (for patients with allergies to alcohol-based solutions):
Administration Timing
- Antiseptic should be applied within 60 minutes before skin incision 1
- For agents requiring longer administration time (e.g., fluoroquinolones, vancomycin), begin administration within 120 minutes before incision 1
Application Technique
- Apply antiseptic solution thoroughly to the surgical site
- Allow sufficient drying time according to manufacturer's instructions
- For optimal results, use a fascial abdominal wound protector during surgery 1
Special Considerations
- For orthopedic surgeries, especially shoulder procedures where Cutibacterium acnes is a concern, consider additional preoperative measures:
Important Notes
- Clindamycin alone is not recommended as a primary preoperative skin antiseptic agent
- If clindamycin is used topically for other purposes, apply a thin film to affected areas 5
- The evidence strongly supports chlorhexidine-alcohol as the most effective preoperative skin antiseptic 1, 2
Surgical Site Infection Prevention Bundle
For comprehensive SSI prevention:
- Use alcohol-based chlorhexidine for skin preparation
- Administer appropriate systemic antibiotic prophylaxis
- Consider wound protectors for abdominal surgery
- Change gloves and instruments before wound closure
- Maintain normothermia and control blood glucose during surgery 1
