Differential Diagnosis for Pain at Top of Shoulder Bone

Single Most Likely Diagnosis

  • Subacromial Bursitis or Rotator Cuff Tendinitis: These conditions are common causes of pain at the top of the shoulder bone due to inflammation of the bursa or tendons surrounding the shoulder joint. Activities that involve repetitive overhead motions can exacerbate these conditions.

Other Likely Diagnoses

  • Acromioclavicular Joint Arthritis: Arthritis in the acromioclavicular joint, which connects the acromion (part of the shoulder blade) to the clavicle (collarbone), can cause pain at the top of the shoulder.
  • Shoulder Impingement Syndrome: This condition occurs when the shoulder blade puts pressure on the underlying soft tissues when you lift your arm away from your body, leading to pain and limited mobility.
  • Muscle Strain: Strains in the muscles around the shoulder, such as the trapezius or deltoid, can cause pain in the area.

Do Not Miss Diagnoses

  • Thoracic Outlet Syndrome: Although less common, this condition involves compression of the nerves and/or blood vessels that pass into the thoracic outlet (the space between your collarbone and first rib), which can cause shoulder pain, among other symptoms.
  • Referred Pain from the Cervical Spine: Sometimes, pain from the cervical spine (neck) can be referred to the shoulder area, including the top of the shoulder bone. Conditions like cervical spondylosis or a herniated disc could be responsible.
  • Pancoast Tumor: A rare type of lung cancer that grows in the upper part of the lung and can invade nearby structures, including ribs, vertebrae, and nerves, causing shoulder pain.

Rare Diagnoses

  • Osteonecrosis of the Humeral Head: A condition where the blood supply to the humeral head (the top of the upper arm bone) is interrupted, leading to bone death and potentially causing pain in the shoulder area.
  • Scapular Fracture: Although fractures of the scapula (shoulder blade) are uncommon, they can occur due to trauma and cause pain at the top of the shoulder bone.
  • Infection or Osteomyelitis: Infections of the bone or soft tissues around the shoulder can cause pain and require prompt medical attention to prevent serious complications.
