Differential Diagnosis for Right Foot Pain at the Top/Side of the Foot

Single Most Likely Diagnosis

  • Stress Fracture: This is a common cause of foot pain, especially in athletes or individuals who have recently increased their physical activity. The top and side of the foot are common locations for stress fractures, which occur due to repetitive stress on the bones.

Other Likely Diagnoses

  • Peroneal Tendinitis: Inflammation of the peroneal tendons, which run along the side of the foot, can cause pain in this area. This condition is often seen in individuals who participate in sports that involve repetitive ankle motion.
  • Cuboid Syndrome: This condition involves inflammation of the cuboid bone and surrounding tissues, located on the outside of the foot. It can cause pain on the top and side of the foot, especially during weight-bearing activities.
  • Morton's Neuroma: A benign growth of nerve tissue between the toes can cause pain on the top of the foot, especially between the third and fourth toes.

Do Not Miss Diagnoses

  • Compartment Syndrome: A serious condition that occurs when pressure within the muscle compartments of the foot increases, compromising blood flow. It can cause severe pain, especially with activity, and requires prompt medical attention to prevent permanent damage.
  • Osteomyelitis: A bone infection that can cause severe pain, redness, and swelling in the affected area. It is a medical emergency that requires prompt treatment with antibiotics.
  • Deep Vein Thrombosis (DVT): A blood clot in the deep veins of the foot or leg can cause pain, swelling, and warmth in the affected area. It is a potentially life-threatening condition that requires immediate medical attention.

Rare Diagnoses

  • Erythromelalgia: A rare disorder that causes burning pain, heat, and redness in the feet and hands. It can be primary (inherited) or secondary (associated with another condition).
  • Freiberg's Disease: A rare condition that affects the metatarsal bones in the foot, causing pain and stiffness, especially in adolescents.
  • Gout: A type of arthritis that can cause sudden, severe pain and swelling in the joints, including those in the foot. It is more common in men and can be triggered by certain foods or medications.
