Albuterol Dosing with Nebulized N-Acetylcysteine
When administering nebulized N-acetylcysteine (NAC), albuterol (Ventolin) should be given at a dose of 2.5-5 mg every 20 minutes for 3 doses, then 2.5-10 mg every 1-4 hours as needed, and can be mixed in the same nebulizer with NAC. 1

Administration Technique
- Dilute albuterol in a minimum of 2-3 mL of saline solution for adequate nebulization 2

Co-administration with NAC
NAC can be safely co-administered with albuterol in the same nebulizer. This combination has been studied in several clinical contexts:
Respiratory support in mechanically ventilated patients: The combination of nebulized NAC and albuterol has been shown to be effective in reducing airway secretion density 3, 4
Inhalation injury management: Nebulized heparin with NAC and albuterol has been associated with reduced duration of mechanical ventilation in burn patients with inhalation injuries 5
Pediatric applications: Nebulized NAC (2 mL of 10% solution in 2:5 dilution) has been used every six hours to liquefy airway secretions in pediatric patients 6
Important Considerations
Monitoring
- Monitor for tachycardia, which is more common with routine nebulization of albuterol with NAC (25.9%) compared to on-demand nebulization (12.5%) 4
- Watch for agitation, which can occur in approximately 4.3% of patients receiving routine nebulization 4
- Monitor oxygen saturation continuously during treatment 2
Cautions
- The combination may increase the risk of adverse events, particularly tachyarrhythmias and agitation, when used routinely rather than on-demand 4
- Consider on-demand nebulization rather than routine scheduled nebulization to minimize adverse effects while maintaining efficacy 4
Clinical Application
For patients requiring both NAC and bronchodilation:
Prepare the nebulizer with both medications:
- Standard albuterol dose (2.5-5 mg for adults)
- Prescribed NAC dose
- Adequate saline dilution (minimum 3 mL total solution)
Administer via nebulizer with oxygen as the preferred driving gas
Monitor for clinical response and adverse effects
