Maximum Dose of Simvastatin
The maximum recommended dose of simvastatin is 40 mg once daily for new patients, while the 80 mg daily dosage is restricted only to patients who have been taking this dose chronically (for 12 months or more) without evidence of muscle toxicity. 1
Current FDA Recommendations
The FDA has placed significant restrictions on simvastatin dosing due to safety concerns:
- The 40 mg daily dose is now considered the maximum recommended dose for new patients 1
- The 80 mg daily dose should only be continued in patients who:
- Simvastatin 80 mg should NOT be initiated in new patients, including those already taking lower doses 2
Rationale for Dose Restriction
The dose restriction is primarily based on safety concerns:
- Higher doses of simvastatin are associated with increased risk of myopathy, particularly among patients with reduced kidney function 2

- While higher doses provide additional LDL-C reduction, the risk-benefit ratio becomes unfavorable at the 80 mg dose for most patients
Dose Adjustments with Concomitant Medications
Several medications require simvastatin dose limitations when used concomitantly:
- With amiodarone or amlodipine: Do not exceed 20 mg daily 2, 1
- With verapamil, diltiazem, or dronedarone: Do not exceed 10 mg daily 2, 1
- With lomitapide: Reduce simvastatin dose by 50%, not exceeding 20 mg daily 1
- Simvastatin is contraindicated with cyclosporine or gemfibrozil 2
Alternative Approaches for High-Intensity Statin Therapy
For patients requiring high-intensity statin therapy who cannot use simvastatin 80 mg:
- Consider atorvastatin 40-80 mg or rosuvastatin 20-40 mg, which are classified as high-intensity statin therapy 2
- These alternatives can achieve LDL-C reductions of ≥50% 2
- For patients unable to achieve LDL-C goals with simvastatin 40 mg, the FDA recommends prescribing alternative LDL-C-lowering treatment 1
Special Populations
- In patients with severe renal impairment (CrCl 15-29 mL/min), the recommended starting dose is 5 mg once daily 1
- In patients with diabetes and CKD, lower doses with combination therapy may be preferable to high-dose statin monotherapy 2
Clinical Monitoring
When prescribing simvastatin:
- Assess LDL-C as early as 4 weeks after initiating therapy 1
