Calcium Score and Pulmonary Artery Dilation Assessment
A calcium score study cannot determine pulmonary artery dilation. Calcium scoring is specifically designed to quantify coronary artery calcification, not to evaluate pulmonary artery dimensions or dilation 1.
What Calcium Scoring Evaluates
Calcium scoring is a specialized CT technique that:
- Quantifies coronary artery calcification as a marker of atherosclerotic disease burden
- Is typically performed using electron beam or multidetector CT with prospective ECG-gating
- Uses standardized protocols with 2.5-3.0mm slice thickness and 120 kVp 1
- Produces an Agatston score that correlates with coronary heart disease risk
- Can be reported using quantitative scores or qualitative visual assessment (none, mild, moderate, or heavy/severe) 1
Pulmonary Artery Assessment
For pulmonary artery dilation assessment:
- A standard contrast-enhanced chest CT or CT pulmonary angiography is required
- Specific measurements of the main pulmonary artery diameter are needed
- Diagnostic thresholds include:
- Main pulmonary artery diameter >29.5mm or >31.5mm
- Ratio of main pulmonary artery diameter to ascending aorta diameter >1.0 1
These measurements have sensitivities of 71%, 52%, and 71% and specificities of 79%, 90%, and 75%, respectively, for detecting pulmonary hypertension 1.
Clinical Implications
When pulmonary artery dilation is detected on imaging:
- It correlates with increased pulmonary artery pressure
- May indicate pulmonary hypertension, which requires specific diagnostic workup
- In fibrotic lung disease, retractile forces on the mediastinum may cause dilation of the main pulmonary artery 1
- Requires hemodynamic monitoring for accurate diagnosis and management of pulmonary hypertension 1
Important Distinctions
While both coronary calcium scoring and pulmonary artery assessment provide valuable diagnostic information, they:
- Evaluate different anatomical structures
- Use different measurement techniques
- Have different clinical implications
- May require different imaging protocols
Recommendations for Proper Assessment
If pulmonary artery dilation is suspected:
- Order a contrast-enhanced chest CT or CT pulmonary angiography
- Request specific measurement of pulmonary artery dimensions
- Consider the ratio of pulmonary artery to aorta diameter
- Follow up with appropriate cardiopulmonary testing if dilation is identified
Calcium scoring alone is insufficient for this purpose as it is designed specifically to evaluate coronary calcification as a marker of atherosclerotic disease.
