Reconstitution of Ceftriaxone 1g Vial
To reconstitute a ceftriaxone 1g vial, add 9.6 mL of appropriate diluent for intravenous administration or 3.6 mL for intramuscular administration. 1
Intravenous Administration
Materials Needed:
- Ceftriaxone 1g powder vial
- Appropriate diluent (sterile water for injection, 0.9% sodium chloride, or 5% dextrose)
- 10 mL syringe
- Needle
- IV administration set (if giving as infusion)
Reconstitution Steps:
- Add 9.6 mL of diluent to the 1g vial 1
- Shake well until completely dissolved
- The resulting concentration will be approximately 100 mg/mL
- Administer by IV infusion over 30 minutes 1
Storage After Reconstitution:
- Room temperature (25°C): Stable for 2 days
- Refrigerated (4°C): Stable for 10 days 1
Intramuscular Administration
Materials Needed:
- Ceftriaxone 1g powder vial
- Appropriate diluent (sterile water for injection, 0.9% sodium chloride, or 5% dextrose)
- 5 mL syringe
- Needle for reconstitution
- Needle for injection
Reconstitution Steps:
- For standard 250 mg/mL concentration: Add 3.6 mL of diluent to the 1g vial
- For higher 350 mg/mL concentration: Add 2.1 mL of diluent to the 1g vial 1
- Shake well until completely dissolved
- Withdraw entire contents into syringe
- Inject deep into a large muscle (e.g., gluteal muscle)
Storage After Reconstitution:
- Room temperature (25°C): Stable for 24 hours
- Refrigerated (4°C): Stable for 3 days 1
Important Considerations
Compatibility:
- Do NOT use diluents containing calcium, such as Ringer's solution or Hartmann's solution, as particulate formation can result 1
- Incompatible with vancomycin, amsacrine, aminoglycosides, and fluconazole in admixtures 1
Administration Warnings:
- For IM administration, inject well within the body of a relatively large muscle
- Aspirate before injection to avoid inadvertent intravascular administration 1
- For IV administration, never administer as a bolus; always infuse over 30 minutes 1
Stability:
- The color of reconstituted solution ranges from light yellow to amber depending on storage time, concentration, and diluent used 1
- Color change within this range does not indicate loss of potency 1
