Human Rabies Immunoglobulin (HRIG) Dosing for a 50kg Individual
For a 50kg individual, the recommended dose of Human Rabies Immunoglobulin (HRIG) is 1000 IU (20 IU/kg × 50kg), which equals approximately 6.65 mL (0.133 mL/kg × 50kg) of standard HRIG preparation. 1
Administration Protocol
Dosing Calculation
- HRIG dose is calculated at 20 IU/kg body weight for all age groups 1
- For a 50kg person: 20 IU/kg × 50kg = 1000 IU total dose
- Using standard HRIG concentration (150 IU/mL): 1000 IU ÷ 150 IU/mL = 6.65 mL
Administration Technique
Wound Infiltration (Priority)
- If anatomically feasible, infiltrate the full dose of HRIG thoroughly in and around all wounds
- Ensure complete infiltration of all bite/exposure sites
- Use a small gauge needle for careful infiltration to avoid tissue damage
Remaining Volume
- Any HRIG that cannot be infiltrated into wounds should be administered intramuscularly
- IM injection site must be distant from vaccine administration site
- Preferred IM site: gluteal area (if not used for vaccine)
Critical Administration Guidelines
- HRIG must be administered only once at the beginning of post-exposure prophylaxis 1
- If not given at initiation of treatment, HRIG can be administered up to day 7 after the first vaccine dose 1
- Beyond day 7, HRIG is not indicated as antibody response to vaccine is presumed to have occurred 1
- HRIG should never be administered:
- In the same syringe as the vaccine
- At the same anatomical site as the first vaccine dose
- In excess of the recommended dose (may suppress active antibody production) 1
Common Pitfalls to Avoid
Underdosing at Wound Sites
- Failure to thoroughly infiltrate wounds with adequate HRIG has been associated with rare post-exposure prophylaxis failures 1
- Prioritize complete wound infiltration over systemic administration
Incorrect Administration Site
- Never administer HRIG in the same anatomical site as the first vaccine dose
- Subsequent vaccine doses can be given at the same site as HRIG if necessary 1
Delayed Administration
- HRIG effectiveness decreases with time after exposure
- Administer as soon as possible after exposure for optimal protection
Exceeding Recommended Dose
- Giving more than 20 IU/kg may suppress active antibody production 1
- Calculate dose precisely based on actual body weight
Special Considerations
- For multiple or extensive wounds where the calculated HRIG volume is insufficient for complete infiltration, some experts suggest diluting HRIG with sterile saline to ensure adequate volume for infiltration 2
- Recent research suggests local wound infiltration alone without systemic IM administration may be effective in some cases, but this approach is not yet part of standard guidelines 3, 4
Remember that proper HRIG administration combined with appropriate rabies vaccination is nearly 100% effective in preventing rabies, which is otherwise almost universally fatal once clinical symptoms develop.
