Differential Diagnosis for Upper and Lower Respiratory Symptoms with Cough and Fever

Single Most Likely Diagnosis

  • Acute Bronchitis: This is often the most common diagnosis for patients presenting with cough and fever, especially if the symptoms are acute in onset. It is usually caused by a viral infection.

Other Likely Diagnoses

  • Influenza: Given the symptoms of cough and fever, influenza should be considered, especially during flu season. It can affect both the upper and lower respiratory tracts.
  • Pneumonia: This can present with similar symptoms, including cough and fever, and can be caused by bacterial, viral, or fungal pathogens.
  • Asthma Exacerbation: If the patient has a history of asthma, an exacerbation could present with cough and fever, among other symptoms.
  • Chronic Obstructive Pulmonary Disease (COPD) Exacerbation: For patients with COPD, an exacerbation could also present with these symptoms.

Do Not Miss Diagnoses

  • Pneumocystis jirovecii Pneumonia (PCP): Especially in immunocompromised patients, PCP can present with non-specific symptoms including cough and fever, and is critical not to miss due to its high mortality if untreated.
  • Tuberculosis (TB): Although less common in some regions, TB can present with chronic cough and fever, and is crucial to diagnose due to its public health implications and the need for specific treatment.
  • Lung Abscess: This condition can present with fever, cough, and other respiratory symptoms, and requires prompt antibiotic treatment.
  • Severe Acute Respiratory Syndrome (SARS) or COVID-19: In the appropriate epidemiological context, these viral infections can present with cough and fever and have significant public health implications.

Rare Diagnoses

  • Cystic Fibrosis: Although more commonly diagnosed in childhood, cystic fibrosis can present in adulthood with recurrent respiratory infections, among other symptoms.
  • Bronchiectasis: This condition involves permanent dilation of parts of the airways and can cause chronic cough and recurrent infections.
  • Sarcoidosis: A systemic disease that can affect the lungs, presenting with cough, fever, and other respiratory symptoms, although it often has a more chronic course.
  • Histoplasmosis or Coccidioidomycosis: Fungal infections that can cause respiratory symptoms, including cough and fever, more commonly in specific geographic regions.
