Uremic encephalopathy can cause a range of neurological symptoms, including twitching or jerking movements, due to the accumulation of uremic toxins that affect brain function.

Electrolyte Imbalance: Electrolyte disturbances, such as alterations in calcium, magnesium, or potassium levels, are common in ESRD patients and can cause muscle twitching or jerking.
Medication Side Effects: Certain medications used in the management of ESRD, such as those for hypertension or anemia, can have side effects that include neurological symptoms like twitching.
Vitamin Deficiencies: Deficiencies in vitamins such as B12 or B6 can occur in ESRD patients due to dietary restrictions or dialysis-related losses, leading to neurological symptoms.

Hypocalcemia: Although less common, hypocalcemia can be life-threatening and is known to cause tetany, which includes muscle twitching or cramps. In the context of ESRD, hypocalcemia can be due to secondary hyperparathyroidism or vitamin D deficiency.
Dialysis Disequilibrium Syndrome: This is a rare but potentially life-threatening condition that occurs during or after dialysis, especially in patients new to dialysis. It can cause neurological symptoms, including seizures or muscle twitching, due to rapid changes in urea and electrolyte levels.

Aluminum Toxicity: Historically, aluminum toxicity was a concern in ESRD patients due to the use of aluminum-containing phosphate binders. While less common with current practices, it can still occur and leads to a condition known as dialysis dementia, which includes neurological symptoms.
Wernicke's Encephalopathy: This is a rare condition caused by thiamine deficiency, which can occur in malnourished patients, including those with ESRD. It presents with neurological symptoms, including muscle weakness and twitching, but is less directly related to the renal condition itself.
