Pharmacokinetics of Rifampicin

Autoinduction Effect
Rifampicin induces its own metabolism through a process called autoinduction:

Clinical Implications
The clearance time of rifampicin has important clinical implications:
Drug Interactions:
- Rifampicin is a potent inducer of cytochrome P450 enzymes (particularly CYP3A4) and drug transporters like P-glycoprotein 1
- These interactions continue for approximately 2 weeks after discontinuation 1
- When rifampicin is discontinued, concentrations of many other drugs used by the patient will increase as the induction wears off 1
Special Populations:
Important Considerations
When planning to start other medications after rifampicin therapy, clinicians should wait at least 2 weeks after the last dose of rifampicin before starting drugs that are significantly affected by enzyme induction 1
Protease inhibitors and NNRTIs for HIV treatment should not be started until at least 2 weeks after discontinuing rifampicin to avoid adverse effects on drug metabolism 6
Rifampicin can compete with bilirubin and other substances for biliary excretion, potentially causing transient and reversible increased bilirubin values 2
