Differential Diagnosis for Neck Swelling and Isolated Elevated Beta 2 Globulin
Single Most Likely Diagnosis
- Multiple Myeloma: This is a plasma cell disorder characterized by the proliferation of malignant plasma cells in the bone marrow. The isolated elevation of Beta 2 Globulin (also known as Beta 2 Microglobulin) is a significant marker for this condition, especially when considered alongside symptoms like neck swelling, which could indicate lymphadenopathy or a plasmacytoma. Elevated Beta 2 Microglobulin levels are associated with a poorer prognosis and are used as a prognostic factor in multiple myeloma.
Other Likely Diagnoses
- Lymphoma: Both Hodgkin and non-Hodgkin lymphoma can present with lymphadenopathy (swelling of the lymph nodes), which might be perceived as neck swelling. While Beta 2 Microglobulin is more specifically associated with multiple myeloma, lymphomas can also lead to elevated levels, particularly in more advanced stages.
- Chronic Lymphocytic Leukemia (CLL): CLL is a type of cancer that affects the blood and bone marrow, characterized by the production of an excessive number of immature white blood cells. Elevated Beta 2 Microglobulin levels can be seen in CLL and are associated with a worse prognosis.

- Human Immunodeficiency Virus (HIV) Infection: Although less directly linked to isolated Beta 2 Microglobulin elevation, HIV infection can lead to a variety of hematologic abnormalities and malignancies, including lymphoma. - Tuberculosis: TB can cause lymphadenopathy and, in rare cases, might be associated with changes in Beta 2 Microglobulin levels, although this is not a primary diagnostic marker.
Rare Diagnoses
- Castleman Disease: A rare disorder that involves an overgrowth of cells in the body's lymph nodes. It can present with lymphadenopathy and might be associated with abnormal protein levels, including potentially Beta 2 Microglobulin, although this is less common.
- POEMS Syndrome: A rare, paraneoplastic syndrome associated with a plasma cell disorder. It is characterized by Polyneuropathy, Organomegaly, Endocrinopathy, Monoclonal gammopathy, and Skin changes. Elevated Beta 2 Microglobulin could be seen in the context of the monoclonal gammopathy component of this syndrome.
