Differential Diagnosis for Illness with Sore Throat and Respiratory Symptoms
Single Most Likely Diagnosis
- Viral Upper Respiratory Infection (URI): This is the most likely diagnosis given the progression of symptoms from a sore throat to respiratory symptoms like congestion, productive cough, and runny nose without fever. The lack of improvement over a week is consistent with the natural course of many viral URIs.
Other Likely Diagnoses
- Allergic Rhinitis: Symptoms such as congestion, runny nose, and cough could be indicative of allergic rhinitis, especially if the patient is exposed to allergens and has a history of allergies.
- Sinusitis: Given the persistence of symptoms like congestion and productive cough, sinusitis (either viral or bacterial) is a possible diagnosis, especially if there are signs of facial pain or headache.
- Influenza: Although the absence of fever makes this less likely, some cases of influenza can present without fever, especially in certain populations like the elderly or those who are immunocompromised.
Do Not Miss Diagnoses
- Pertussis (Whooping Cough): This bacterial infection can present with a persistent cough and is a critical diagnosis not to miss due to its potential for severe complications, especially in infants and young children, and its contagious nature.
- Tuberculosis (TB): Although less common, TB can present with chronic cough and should be considered, especially in individuals with risk factors such as exposure history, immunocompromised status, or recent travel to endemic areas.
- Lung Cancer: In adult patients, especially those with a history of smoking, a persistent cough could be a symptom of lung cancer, making it a critical diagnosis not to miss.
Rare Diagnoses
- Sarcoidosis: This autoimmune disease can cause respiratory symptoms among others and should be considered in patients with persistent, unexplained symptoms.
- Histoplasmosis or Coccidioidomycosis: Fungal infections that can cause respiratory symptoms, particularly in individuals who have traveled to or live in areas where these fungi are common.
- Relapsing Polychondritis: A rare autoimmune disorder that can cause respiratory symptoms due to cartilage inflammation, among other systemic symptoms.
