Recommended Dosage of Tinidazole (Tinnicar)
The recommended dosage of tinidazole (Tinnicar) varies by indication, with the standard adult dose being 2 g orally in a single dose for trichomoniasis and giardiasis, 2 g daily for 3 days for intestinal amebiasis, and 2 g daily for 3-5 days for amebic liver abscess. 1
Dosage by Indication
Trichomoniasis
- Adults and adolescents: 2 g orally in a single dose 1
- Should be taken with food to minimize gastrointestinal side effects
- Sexual partners should be treated simultaneously with the same dose 2
Giardiasis
- Adults: 2 g orally in a single dose 1
- Children >3 years: 50 mg/kg (up to 2 g) as a single dose 1
- Should be taken with food
Amebiasis
Intestinal amebiasis:
Amebic liver abscess:
Bacterial Vaginosis
- Adults: Either 2 g orally once daily for 2 days OR 1 g orally once daily for 5 days 1
- Not studied in pregnant patients for this indication 1
Administration Guidelines
- Take with food to minimize epigastric discomfort and other gastrointestinal side effects 1
- Food does not affect the oral bioavailability of tinidazole 1
- Avoid alcoholic beverages during treatment and for 3 days afterward 1
For Patients Unable to Swallow Tablets
Tinidazole tablets may be crushed in artificial cherry syrup:
- Pulverize tablets with mortar and pestle
- Add approximately 10 mL of cherry syrup and mix until smooth
- Transfer to graduated amber container
- Rinse mortar with cherry syrup to transfer any remaining drug
- Final volume should be 30 mL
- Suspension is stable for 7 days at room temperature
- Shake well before each administration 1
Clinical Efficacy Considerations
Tinidazole has demonstrated high efficacy in the treatment of protozoal infections with cure rates generally exceeding 90% 3. In comparative studies, tinidazole has shown equivalent or superior efficacy to metronidazole:
- For intestinal amebiasis, tinidazole demonstrated significantly higher cure rates (90-96.5%) compared to metronidazole (53.3-55.5%) when both were administered as 2 g once daily for 3 days 4, 5
- Tinidazole's longer half-life (12.3 hours vs. 7.3 hours for metronidazole) may contribute to its efficacy in single-dose regimens 6, 3
Common Side Effects
- Bitter taste
- Nausea
- Abdominal discomfort
- Anorexia
- Vomiting
- Fatigue 3
Special Considerations
- No dose adjustment necessary based on race, sex, or renal function 3
- Not recommended in patients with severe hepatic impairment (Child-Pugh class C) 3
- For treatment failures in trichomoniasis, consider metronidazole 500 mg twice daily for 7 days 2
Tinidazole's pharmacokinetic profile, with its longer half-life and higher serum levels at 48-72 hours post-dose compared to metronidazole, makes it particularly suitable for once-daily dosing regimens 7.
