Counseling Points for Ciprofloxacin
When counseling patients about ciprofloxacin, inform them that it should be taken with plenty of fluids, can be taken with or without food (but not with dairy products alone), and to complete the full course of treatment even if they feel better before it's finished.
Administration Guidelines
- Take ciprofloxacin exactly as prescribed, at about the same time each day (morning and evening) 1
- Swallow tablets whole; do not split, crush, or chew 1
- Can be taken with or without food, but:
- Take 2 hours before or 6 hours after:
- Antacids containing magnesium, calcium, aluminum, iron, or zinc
- Sucralfate
- Didanosine
- Multivitamins containing minerals 1
- Drink plenty of fluids while taking ciprofloxacin 1
Important Safety Warnings
- Risk of tendon inflammation and rupture, especially in:
- Older patients (over 60 years)
- Patients taking corticosteroids
- Patients with kidney, heart, or lung transplants 1
- Stop taking and rest at first sign of tendon pain, swelling, or inflammation 1
- May cause dizziness and lightheadedness; use caution when driving or operating machinery 1
- Serious central nervous system effects can occur, including:
- Seizures
- Confusion
- Hallucinations
- Depression
- Rarely, suicidal thoughts 1
- Severe hypersensitivity reactions can occur, even after first dose 1
- May cause photosensitivity; avoid excessive sun exposure 2
Drug Interactions
- Alert healthcare provider about all medications, including:
- NSAIDs (increased risk of CNS effects and seizures)
- Blood thinners (warfarin) - may enhance anticoagulant effects
- Theophylline - serious and potentially fatal reactions
- Tizanidine - should not be taken with ciprofloxacin
- Glyburide
- Phenytoin
- Products containing caffeine
- Heart rhythm medications
- Antipsychotics
- Tricyclic antidepressants

Special Populations
- Pregnancy: Not recommended during pregnancy unless for life-threatening illness when benefits outweigh risks 3

Treatment Duration
- Do not skip doses or stop taking ciprofloxacin even if you begin to feel better 1
- Continue for at least 2 days after signs and symptoms of infection have disappeared 2
- For anthrax exposure, the full 60-day course is critical due to potential persistence of spores 3
Potential Side Effects
- Common side effects include:
- Report immediately to healthcare provider:
- Skin rash, itching
- Severe diarrhea (may indicate C. difficile infection)
- Tendon pain or swelling
- Neurological symptoms (confusion, hallucinations, seizures)
- Signs of allergic reaction (difficulty breathing, facial swelling) 1
Absorption Information
- Oral ciprofloxacin is rapidly and well absorbed from the gastrointestinal tract
- Maximum serum concentrations are attained 1-2 hours after oral dosing
- May not be fully absorbed if vomiting or ileus is present 3
