Differential Diagnosis for Viruses Causing Similar Symptoms in Adults

Single Most Likely Diagnosis

  • Influenza: This is often the first consideration for adults presenting with acute respiratory symptoms such as cough, fever, and fatigue, especially during flu season. The high prevalence and contagious nature of influenza make it a prime suspect.

Other Likely Diagnoses

  • Respiratory Syncytial Virus (RSV): Although more commonly associated with severe illness in infants and young children, RSV can also cause significant respiratory illness in adults, especially older adults or those with compromised immune systems.
  • Adenovirus: Known for causing a range of illnesses from common cold symptoms to gastroenteritis, adenovirus infections can present similarly to influenza in adults and are a common cause of respiratory infections.
  • Human Metapneumovirus: This virus is another cause of respiratory tract infections in adults and can mimic the symptoms of influenza, making it a likely diagnosis in the differential.

Do Not Miss Diagnoses

  • COVID-19: Given its global impact and potential for severe disease, COVID-19 is a critical diagnosis not to miss. It presents with a wide range of symptoms, including fever, cough, and shortness of breath, similar to other respiratory viruses.
  • Hantavirus: While less common, hantavirus infections can cause severe respiratory symptoms (Hantavirus Pulmonary Syndrome) and have a high mortality rate if not recognized and treated promptly.
  • SARS-CoV: Although rare, given the global interconnectedness, missing a diagnosis of SARS or other severe coronaviruses could have significant individual and public health implications.

Rare Diagnoses

  • Cytomegalovirus (CMV): Typically causes more severe disease in immunocompromised individuals, but can cause mild to moderate illness in healthy adults, including symptoms like fever and fatigue.
  • Parainfluenza Virus: More commonly associated with croup in children, parainfluenza viruses can cause respiratory infections in adults, though this is less common.
  • Coronaviruses other than SARS-CoV, SARS-CoV-2, and MERS-CoV: Other coronaviruses can cause mild to moderate respiratory illness in adults, similar to the common cold, but are less frequently identified as the cause of significant disease.
